Incident Narrative
LANDOWNER REPORTED AN OIL LEAK AND THE PIPELINE OPERATOR RESPONDED TO THE LEAK SITE. UPON ARRIVAL, THE PIPELINE OPERATOR SAW THE LEAK AND DETERMINED THAT THE CRUDE OIL WAS DRIPPING FROM THE PACKING GLAND NUT. THE PIPELINE OPERATOR TIGHTENED THE PACKING NUT TO STOP THE LEAK. THE LEAK WAS REPORTED TO THE NECESSARY AND REGULATORY AGENCIES WITHIN 24 HOURS OF THE INCIDENT. THE ESTIMATED VOLUME SPILLED DUE TO THE LEAK IS 15 GALLONS. THE VALVE WAS OPERATED AND DEVELOPED A SLOW LEAK. PREVENTION ACTION PLAN IS TO CHECK AND MAINTAIN VALVES IN ACCORDANCE TO OUR O&M MANUAL IN SIMILAR LOCATIONS TO PREVENT THE ISSUE.
