"ON 8/10/2024 KM EMPLOYEES IDENTIFIED A PRODUCT STAIN ON THE CHIME OF TANK 80-16. THE TANK HAS BEEN DRAINED AND STRIPPED. THE TANK FLOOR WAS NDT TESTED, THE WELDS WERE VACUUM TESTED AND IDENTIFIED A WELD FAILURE DUE TO LACK OF FUSION THAT CAUSED THE LEAK. THE WELD WAS REPAIRED PER API 650/653."
