ON 11/4/2022, THE OPERATIONS MANAGER WAS MAKING A ROUND CHECKING LIGHTING IN THE FACILITY. UPON MAKING HIS ROUNDS, HE OBSERVED A SIGNIFICANT PUDDLE OF CRUDE AROUND PUMP E. AFTER FURTHER INVESTIGATION, IT WAS DETERMINED THAT A SEAL HAD FAILED AND CREWS WERE DISPATCHED TO START THE CLEAN-UP PROCESS.
