Incident Narrative

ON 11/30/2022 AT 03:00, THE OPERATOR DISCOVERED A PINHOLE LEAK ON A 10-INCH LINE INSIDE THE ROTATION MANIFOLD. THE LINE WAS NOT OPERATING AT THE TIME BUT WAS ISOLATED WHILE CLEANUP AND REPAIRS TOOK PLACE. APPROXIMATELY 5 GALLONS OF CRUDE OIL WAS RELEASED DUE TO AN INTERNAL CORROSION PINHOLE AND WAS CLEANED UP USING SORBENT PADS AND BOOM. THE FAILURE WAS REPAIRED WITH A MECHANICAL CLAMP. UAT SCANS CONTINUE OF THE OTHER MANIFOLD PIPING AND A REVIEW WILL BE CONDUCTED TO IDENTIFY MITIGATIVE MEASURES.
