"THE CAUSE OF THE RELEASE WAS A PIG RECEIVER 6"" BLOCK VALVE, ON THE OUTLET SIDE OF RECEIVER. THIS VALVE LEAKED BETWEEN FLANGES OF BLOCK VALVE BODY, CAUSING RELEASE. THE BODY SEALING O-RING FAILED DUE TO IMPROPER ASSEMBLY AT MANUFACTURER. THE BLOCK VALVE WAS IN SERVICE A TOTAL OF 102 DAYS."
