Incident Narrative
A RELEASE OCCURRED ON THE EUGENE ISLAND METER SKID AT GB 128 DUE TO LEAKING THREADS ON THE PSV 280 PROTECTING THE METER RUN. THE METER SKID WAS ISOLATED TO PREVENT FURTHER LOSS OF PRODUCT. APPROXIMATELY 10 GALLONS OF CRUDE OIL WAS CAPTURED IN SECONDARY CONTAINMENT.
