COLUMBIA GAS OF OHIO, INC. (COLUMBIA) WAS NOTIFIED BY ITS SCADA AND BY THE GENERAL PUBLIC OF A LEAK AT THE INTERSECTION OF SR42 AND HANLEY RD. COLUMBIA FOUND A 12"" STEEL MAIN LINE LEAK WHERE A SHORT BARRELED BOLTED COUPLING WITHOUT PULL OUT RESTRAINT FAILED. COLUMBIA SHUT OFF THE LINE FOR SAFETY AND MADE PERMANENT REPAIRS. THIS EVENT RESULTED IN A SERVICE DISRUPTION TO 996 CUSTOMERS. PLEASE CONSIDER THIS REPORT FINAL.
