Incident Narrative
ON AUGUST 2ND AT 11:40 MOUNTAIN, A CREW PERFORMING A REHAB DIG WAS SANDBLASTING THE PIPE IN PREPARATION FOR REPAIR. WHEN THE PIPE COATING WAS REMOVED, A HOLE IN THE PIPE WAS EXPOSED CAUSING A RELEASE OF APPROXIMATELY 15 GALLONS OF GASOLINE. WORK WAS HALTED AND THE SYSTEM WAS SHUT DOWN IN ORDER TO MAKE A REPAIR USING A PLIDCO CLAMP. SUPPLEMENTAL UPDATE TO PART B11. CHANGED TO ""EXPOSED DUE TO EXCAVATION
