Incident Narrative
ON OCTOBER 17, 2023, SFPP RECEIVED ANALYTICAL DATA FOR A SAMPLE THAT WAS TAKEN FROM THE CONCORD STATION TANK CC-10 VALVE BOX ON AUGUST 17, 2023. THE ANALYTICS SHOWED A CHANGE WHEN COMPARED TO HISTORICAL SAMPLES TAKEN FROM THE VALVE BOX. BASED ON THE CHANGE IN ANALYTICAL DATA, SFPP SUSPECTED A LEAK IN OR IN PROXIMITY TO TANK CC-10, AND THREATENED RELEASE NOTIFICATIONS WERE COMPLETED TO CALIFORNIA OFFICE OF EMERGENCY SERVICES AND THE NATIONAL RESPONSE CENTER UNDER REPORT 1382010. SFPP CEASED PRODUCT MOVEMENT IN AND OUT OF TANK CC-10 AND EMPTIED THE TANK AND DRAINED ITS DELIVERY LINE. THE TANK WAS COMPLETELY EMPTIED AND ISOLATED ON OCTOBER 17, 2023. ON DECEMBER 13, 2023, SFPP DISCOVERED A 3 TO 4-INCH CRACK NEXT TO THE FILLET WELD OF THE STRIKER PLATE ON THE CC-10 TANK FLOOR. AN INTERNAL ""EMERGENCY RESPONSE LINE"" CALL WAS INITIATED, WHERE, FOLLOWING THE EXPLANATION OF THE SIZE AND LOCATION OF THE CRACK, THE PARTICIPANTS AGREED THE INFORMATION MET THE CRITERIA FOR CONFIRMED DISCOVERY OF A REPORTABLE RELEASE. THE INSPECTION OF THE TANK FLOOR HAS SHOWN THE CRACK IS A LOCAL FAILURE LIKELY DUE TO MECHANICAL STRESS AT THE STRIKER-PLATE. 2/22/2024 UPDATE: THE KINDER MORGAN INTERNAL INVESTIGATION CONCLUDED THE CRACK IN THE TANK FLOOR WAS LIKELY DUE TO A COMBINATION OF FATIGUE AND MECHANICAL STRESS AT THE EDGE OF THE RECTANGULAR STRIKER PLATE FROM THE FREQUENCY OF TANK ROOF LANDINGS RELATED TO DRAIN-DRY BREAKOUT TANK OPERATIONS. THE CRACK IN THE TANK FLOOR HAS BEEN REPAIRED, AND THE RECTANGULAR STRIKER PLATE HAS BEEN REPLACED WITH A ROUND STRIKER PLATE. 4/23/2024 UPDATE: KINDER MORGAN COMPLETED REPAIRS ON CONCORD TANK CC-10 AND RETURNED THE TANK TO SERVICE ON APRIL 19, 2024.
