Incident Narrative
STATION LOCATED INSIDE METAL BUILDING ON THE CUSTOMER'S PROPERTY (ARCELOR MITTAL STEEL). TAP CONNECTED TO 1/2"" WELDED HALF COUPLING WAS DAMAGED. THE PIPE NIPPLE BROKE AT THE THREADED POINT WHERE IT ENGAGED INTO THE HALF COUPLING. THERE WAS A VIBRATION AT THE STATION THAT CAUSED THE NIPPLE TO CRACK. THE NIPPLE HAD A CAP ON THE OUTLET SIDE OF THE FITTING, WHICH WAS REPLACED WITH A PIPE PLUG. THE PLUG IS THICKER AT THE THREADED POINT AND WILL SERVE THE SAME PURPOSE.
