Incident Narrative
"AT 15:18 EVERLINE CONTROL ROOM RECEIVED A LEAK WARNING ALARM ON THE MAYSVILLE TO WEST TRUNK EXTENSION SEGMENT. AT 15:21 THE CONTROL ROOM NOTIFIED COFFEYVILLE RESOURCES CRUDE TRANSPORTATION, LLC'S (CRCT'S) OPERATIONS SUPERVISOR OF THE ALARM AND BEGAN TO SHUT DOWN THE LINE. UPON FURTHER INVESTIGATION IN THE FIELD, A LEAK WAS IDENTIFIED AT THE 6-8 CROSSOVER NEAR DRUMRIGHT OK. A DRAIN LINE ON THE INCOMING TRAP HAD FAILED AT AN ALL THREAD NIPPLE AND BEGAN RELEASING CRUDE OIL TO THE SURROUNDING AREA. CRCT OPERATIONS WAS ABLE TO CLOSE VALVES ON THE TRAP TO STOP THE LEAK. APPROXIMATELY 5 BARRELS OF CRUDE WAS RELEASED."
