Incident Narrative
"AFTER MAINTENANCE/IMP INSPECTION OF THE E-2 WEST, THE RECOMMISSIONING PROCEDURE CALLED FOR AN ANALYZER TECHNICIAN TO BE AT THE STATION 14 MILES UPSTREAM OF THE STATION WHERE THE FLARE AND MUFFLER WERE LOCATED. BASED ON INTERFACE DETECTION THE PIPELINE TECHNICIANS WOULD KNOW WHEN TO SWAP VALVES FROM THE MUFFLER TO THE FLARE. A DECISION WAS MADE BY THE AREA PIPELINE TECH TO MOVE THE ANALYZER TECH DOWNSTREAM TO THE LOCATION OF THE MUFFLER/FLARE, THEREBY NOT PROVIDING SUFFICIENT TIME TO DETECT INTERFACE AND SWAP TO FLARE. IN LIEU OF HAVING ANALYZER TECH UPSTREAM, AN LEL METER WAS USED TO DETECT INTERFACE ON THE MAINLINE VALVE BYPASS, HOWEVER THE BYPASS VALVE WAS IN THE CLOSED POSITION. THIS CAUSED A MISSED INTERFACE RESULTING IN AN UNANTICIPATED ARRIVAL OF THE INTERFACE, WHICH WAS RELEASED THROUGH THE MUFFLER FOR 2 MINUTES. DURING THE RELEASE, GAS DETECTORS WENT INTO ALARM AND OPERATORS DIVERTED THE FLOW FROM THE MUFFLER TO THE FLARE."
