KERN BELIEVES, AFTER EXCAVATION, THAT A UNDOCUMENTED CONTROL VALVE FAILED CAUSING A SURGE OF CRUDE OIL TO FILL AN UNDOCUMENTED GATHERING LINE DEAD LEG. THIS SUDDEN SURGE OF PRESSURE CAUSED THE DEAD LEG TO RUPTURE THROUGH PINHOLES.
