ON 4/17/2024 AT 9:00 AM CDT, AT MIDLAND TANK FARM, WHILE PERFORMING MAINTENANCE ON FACILITY PIPE A FLASH WAS OBSERVED. ALL MAINTENANCE WORK WAS STOPPED, AND A SAFETY REVIEW WAS PERFORMED. A NEW JOB PLAN WAS WRITTEN TO ADDRESS IN MORE DETAIL SAFELY PERFORMING MAINTENANCE ON ACTIVE PIPE.
