APPROXIMATELY 17.9 GALLONS OF CRUDE OIL WERE RELEASED FROM THE WOLFBONE TO ADVANTAGE 12INCH PIG TRAP. THE CAUSE OF THE RELEASE IS ATTRIBUTED TO INCORRECT OPERATION WHEN AN OPERATOR INADVERTENTLY LEFT A DRAIN VALVE IN THE OPEN POSITION, RESULTING IN A SPRAY. TO PREVENT FUTURE INCIDENTS, THE EMPLOYEE INVOLVED WITH THE INCIDENT WAS RE-TRAINED.
