THE REGULATOR FOR THE PNEUMATIC ACTUATOR FROZEN UP AND CAUSE PRESSURE TO DROP BELOW WHAT WAS NEEDED TO KEEP THE ACTUATOR CLOSED AND IT FAILED OPEN RELEASING GAS THROUGH THE BLOW DOWN VALVE ON THE ESD DISCHARGE BLOWDOWN VALVE(3"") ON 16"" HEADERS.
