Incident Narrative
"THIRD PARTY CONTRACTOR STRUCK PEOPLES GAS SYSTEM'S MAIN DURING HDD OPERATION. UPON ARRIVAL ON SCENE AFTER THE REPORT OF THE DAMAGE, THE OPERATOR DID NOT IDENTIFY ANY PHYSICAL EVIDENCE THAT THE THIRD PARTY PERFORMED SPOT DIGGING TO IDENTIFY THE UTILITIES. PIPELINE WAS TOO DEEP TO DIG UP AND SQUEEZE, SO OPERATOR TURNED OFF CRITICAL VALVE STOPPING FLOW OF GAS. PRETESTED PIPELINE WAS INSTALLED, REPAIRS FINALIZED, AND RESTORATION COMPLETED."
