A CONTRACTOR STRUCK A 4"" PLASTIC GAS MAIN WHILE BORING TO INSTALL A TELECOM LINE. DURING RESPONSE, GAS MIGRATED AND IGNITED BENEATH AN EXCAVATOR BEING USED TO EXPOSE THE PIPELINE. THE LOCATOR INACCURATELY AND INSUFFICIENTLY MARKED THE REQUESTED AREA. ALSO DURING THE INCIDENT, THERE WERE 68 COMMERCIAL OUTAGES.
