Incident Narrative
A SPECTRA ENERGY WELDER WAS REMOVING A PORTION OF ROAD CASING ENCIRCLING THE 30-INCH CARRIER PIPE USING A HOT CUT METHOD WHEN A PIN HOLE LEAK OCCURRED DUE TO THE TORCH COMING IN CONTACT WITH TH CARRIER PIPE. NATURAL GAS WAS RELEASED THROUGH THE PIN HOLE.
