Incident Narrative
A LEAK CALL WAS RECEIVED AT THE COMPANY'S CALL CENTER WHERE UPON INVESTIGATION A CRACK WAS DISCOVERED IN A 30-INCH WELD-OVER FITTING NEAR THE INTERSECTION OF WHERE THE FITTING IS WELDED TO THE PIPE. APPARENT CAUSE IS IDENTIFIED IN SECTION G6.
