Incident Narrative
PIPELINE FAILED DURING A DUCTILE OVERLOAD EVENT. THE RUPTURE WAS CAUSED BY MECHANICAL GOUGES ON THE PIPE FROM A SECOND PARTY CONTRACTOR'S TRACKHOE THAT GOT STUCK IN THE MUD NEAR THE PIPELINE, APPROXIMATELY ONE MONTH PRIOR TO THE INCIDENT.
