"A SUBCONTRACTOR FOR A THIRD PARTY CONTRACTOR WAS BORING TO INSTALL FIBER OPTIC LINES AND BORED THROUGH OPERATOR'S 4 INCH MAINLINE, WHICH WAS PROPERLY MARKED. THIRD PARTY CONTRACTOR ACKNOWLEDGED THAT THEIR SUB-CONTRACTOR DECIDED TO DO A PORTION OF THE JOB WITHOUT BEING INSTRUCTED TO DO SO AND FAILED TO POTHOLE, WHICH CREATED THIS EXCAVATION DAMAGE."
