Incident Narrative

"THE FINAL LABORATORY ANALYSIS REPORT FROM DNV INDICATES THAT THE LEAK OCCURRED ON THE UPSTREAM GIRTH WELD ON THE ELBOW SIDE DUE TO INTERNAL CORROSION. THE CORROSION WAS CONCENTRATED AT THE ELBOW FITTING AT THE HEAT AFFECTED ZONE OF THE GIRTH WELD. EVIDENCE INDICATES THERE WAS A CONDUCTIVE ENVIRONMENT AT THE ELBOW FITTING GIRTH WELD DUE TO THE POTENTIAL FOR WATER TO ACCUMULATE, LOW PH, AND HIGH CHLORIDES."
