CONTRACTOR DAMAGED A 10"" STEEL TRANSMISSION LINE WITH A TRENCHER WHILE PERFORMING AGRICULTURE WORK. THE RELEASE WAS FROM A MECHANICAL PUNCTURE. THE SIZE OF THE MECHANICAL PUNCTURE WAS 0.25 X 0.25 BUT THE PORTAL WILL NOT ALLOW TWO DECIMAL PLACES. CONTRACTOR FAILED TO NOTIFY THE ONE-CALL CENTER.
