Incident Narrative
"A THIRD-PARTY CONTRACTOR WAS INSTALLING ELECTRICAL CONDUIT FOR A NEW RESIDENCE. THE CONTRACTOR HAD LOCATED AND IDENTIFIED THE GAS MAIN, BUT FAILED TO DETERMINE THE CLEARANCE NECESSARY TO AVOID STRIKING THE GAS MAIN. AS THE CONTRACTOR WAS CONTINUING TO REMOVE COVER, THE TEETH OF THE EXCAVATOR BUCKET STRUCK AND SHEARED OFF A SERVICE TEE FOR THE RESIDENCE. THIS STRIKE LED TO THE UNCONTROLLED RELEASE OF GAS AND IGNITED. THE CONTRACTOR'S EXCAVATOR WAS DESTROYED BY THE FIRE, AND SIGNIFICANT DAMAGE OCCURRED TO THE NEW RESIDENCE BEING BUILT, ALONG WITH DAMAGE TO OTHER UTILITIES IN THE AREA."
