"FOLLOWING A SERVICE VALVE REPLACEMENT, ENVIRONMENTAL CONDITIONS ALLOWED FOR RESIDUAL GAS TO BE IGNITED AND RESULTED IN BURNS ON TWO EMPLOYEES. NOTE - FOR QUESTION A4 THE DISTRICT USED 8:30 AM AT THE DIRECTION OF PHMSA. THIS WAS NOT THE TIME THAT THE INCIDENT BECAME REPORTABLE. THAT TIME WAS WHEN THE EMPLOYEE WAS HOSPITALIZED WHICH OCCURED AT 13:30. NOTE - FOR QUESTIONS A7 & A21 THE DISTRICT USED 0.1 AT THE DIRECTION OF PHMSA. ALL GAS RELEASED PRIOR TO THIS INCIDENT WAS INTENTIONAL."
