"SYSTEM CONTROL RECEIVED A RATE OF CHANGE ALARM AT 18:51 ON THE SUCTION PIPING AT CHANDLER COMPRESSOR STATION, ANOTHER ALARM AT 18:53 ON THE CHANDLER TO AC DOWNSTREAM AND ANOTHER ALARM AT 18:55 ON THE CHANDLER STATION SUCTION PIPING. A FIELD EMPLOYEE, WHO LIVES WITHIN MINUTES OF THE CHANDLER COMPRESSOR STATION, HEARD THE VENT AT THE STATION FROM HIS HOME. HE IMMEDIATELY CALLED SYSTEM CONTROL AT 19:00 TO LET THEM KNOW HE WAS IN ROUTE TO CHECK THIS OUT. AT 19:15, EMPLOYEE ARRIVED ON SITE TO CONFIRM WE HAD AN EVENT TAKING PLACE. EMPLOYEE HAD TO CALL FOR ADDITIONAL PERSONNEL, THAT WAS TRAINED ON THIS STATION, TO RESPOND TO ASSIST IN THE EVENT. IT WAS DETERMINED THAT AN O-RING ON THE SHAFT SEAL FAILED DURING THE BELOW FREEZING WEATHER, WHICH INCLUDED ICE AND SNOW, ON THE 8"" PILOT OPERATOR ON THE STATION ESD VENT VALVE ON THE SUCTION HEADER OF THE STATION, CAUSING THE VALVE TO OPEN AND VENT GAS. VALVE WAS ISOLATED AT 19:40. PILOT OPERATOR WAS REPLACED, AND SYSTEM WAS PUT BACK IN SERVICE AT 21:30 THE SAME DAY. PART G REVISED TO G2 PER REQUEST BY MATHEWS/PHMSA."
