Incident Narrative
THE CONTRACTOR DAMAGED A 4"" STEEL LINE WHILE WELDING A SHUT-OFF VALVE DURING THE REPLACEMENT PROCESS OF A CITY TAP. THE DAMAGE RESULTED IN A LEAK AND ONE CUSTOMER OUTAGE. THE CONTRACTOR FAILED TO FOLLOW THE PROCEDURE.
