Incident Narrative
"ON MARCH 27TH, 2025, 11:59 A.M., AN UNEXPECTED RELEASE OF NATURAL GAS WAS OBSERVED AT THE HOLLYWOOD AND VINE FACILITY. THE RELEASE CAME FROM THE LAUNCHER DOOR CLOSURE AND BLOWDOWN STACK OF THE LOADING BARREL. AN ETC EMPLOYEE WAS ONSITE AND WAS ABLE TO QUICKLY ENGAGE THE ASD SYSTEM ALLOWING THE SYSTEM TO BLOCK AND BLEED WITHIN FIVE MINUTES. DURING OUR INTERNAL INVESTIGATION IT WAS DETERMINED THAT THE EMPLOYEE LOADING THE SPHERE PIGS INTO THE LAUNCHER HAD FAILED TO SECURE THE PIG LAUNCHER DOOR CLOSURE ASSEMBLY WHICH RESULTED IN A GAS RELEASE DURING THE BARREL PURGING PROCESS."
