"HARTCO CABLE, INC. (THIRD PARTY CONTRACTOR) BORED INTO A 16-INCH NATURAL GAS STEEL MAIN WHILE INSTALLING CABLE TV AT 1400 NW 86TH STREET IN CLIVE, IOWA ON AUGUST 2, 2023. THE BORING HEAD PUNCTURED A FOUR-INCH BY TWO-INCH HOLE IN THE LOWER THIRD OF THE MAIN THAT WAS AT A SIX-FOOT DEPTH. NO SERVICES WERE INTERRUPTED AND THERE WAS NO IGNITION OR INJURIES. DUE TO THE SIZE AND DEPTH OF THE MAIN, IT TOOK A SIGNIFICANT AMOUNT OF TIME TO REPAIR THE DAMAGE AND 6.771 MCF OF NATURAL GAS WAS RELEASED TO ATMOSPHERE. THE NRC WAS INITIALLY NOTIFIED (REPORT 1375040) ON AUGUST 2 AND A 48-HOUR FOLLOW-UP REPORT WAS SUBMITTED (REPORT 1375241)."
