"2"" POLYETHYLENE GAS MAIN WAS PUNCTURED DURING THE CLEARING OF THE SANITARY SEWER SERVICE BY THIRD PARTY FROM WITHIN THE HOUSE AT 2014 VILLARD AVENUE. GAS ESCAPED INTO THE HOUSE THROUGH THE SEWER SERVICE AND WAS IGNITED BY AN UNDERTERMINED SOURCE OF IGNITION."
