Incident Narrative
CONTRACTOR FOR THE COMPANY WAS PERFORMING DIRECTIONAL BORING WORK ASSOCIATED WITH A SERVICE RENEWAL. THE EXISTING SERVICE WAS CONTACTED BY THE BORE HEAD CAUSING A MALLEABLE ELBOW TO BREAK. GAS MIGRATED INTO AN UNDERGROUND CELLAR RESULTING IN AN EXPLOSION AND FIRE.
