Incident Narrative
ON JUNE 14, 2024, DURING ROUTINE LINE PATROLS, GULF SOUTH DISCOVERED A SMALL LEAK ON ITS BISTINEAU STORAGE FIELD WELL A-11 LINE, REFERRED TO AS INDEX 900-8. THE WELL WAS SHUT IN AND THE PIPE SECTION WAS ISOLATED AND DEPRESSURIZED. THE RESPONSES PROVIDED TO ALL PART C QUESTIONS ARE BASED ON THE PIPE SEGMENT BUT UNTIL EXCAVATION OCCURS, IT IS UNKNOWN IF THE LEAK IS IN THE PIPE BODY OR THE PIPE SEAM. A SUPPLEMENTAL REPORT WILL BE SUBMITTED WHEN REPAIRS ARE COMPLETE.
