Incident Narrative

ON FRIDAY AUGUST, 2 2024 AT 10:59 AM, DIG 32020 WAS BEING EXCAVATED AT MP 1153.5 ON THE IGNACIO TO SUMAS MAINLINE LINE 1400. THIS WORK IS PART OF THE 2024 GOLDENDALE TO WILLARD ANOMALY DIGS. DURING EXCAVATION, PERSONNEL HEARD THE SOUND OF GAS BLOWING THROUGH A HOLE. THE EXCAVATION WAS STOPPED AND IT WAS INVESTIGATED USING GAS DETECTORS. LEL WAS PICKED UP ON GAS MONITORS, AND THE LEAK LOCATION WAS CONFIRMED USING LIQUID LEAK DETECT AGENT. A LEAK CLAMP WAS NOT READILY AVAILABLE, SO THE CREW DEMOBILIZED FOR THE DAY. THE FOLLOWING MORNING, THE EXCAVATION WAS COMPLETED BY HAND WHILE MONITORING FOR LEL. INITIAL INVESTIGATION INDICATES A MANUFACTURING DEFECT IN THE LONG SEAM OF THE DSAW PIPE. A TEMPORARY PLIDCO LEAK CLAMP WAS SUCCESSFULLY INSTALLED ON 8/3/24, AT 10:30 AM. PERMANENT REPAIR BY CUTOUT IS EXPECTED TO BE COMPLETED IN THE FUTURE.
