A CONTRACTOR DROPPED A 1 INCH BY 4 FOOT BY 10 FOOT STEEL STREET PLATE ON AN EXPOSED 20'' IP GAS MAIN. THE STEEL PLATE PUNCTURED THE GAS MAIN AND REMAINED LODGED IN THE PIPELINE UNTIL IT WAS TAKEN OUT OF SERVICE.
