Incident Narrative
ON NOVEMBER 16, 2022 AT 9:06 AM, A THIRD-PARTY CONTRACTOR DAMAGED A 2-INCH MEDIUM PRESSURE (60 PSIG MAOP) PLASTIC MAIN LOCATED AT 1363 CREOLA AXIS LOOP, CREOLA AL. THE CONTRACTOR WAS MOVING A TRACKHOE IN A SOFT AREA OF DIRT AND SUNK THE TRACKS INTO THE UNDERGROUND PIPELINE, CAUSING A RUPTURE. A FIRE IGNITED AND DAMAGED THE TRACKHOE. THIS REPORT WAS SUBMITTED BY PORTAL ON DECEMBER 16, 2022 BUT HAD TO BE RESUBMITTED DUE TO TECHNICAL ISSUES IN THE PORTAL.
