"WHILE ATTEMPTING TO REMOVE A 1 1/4"" X 1"" STEEL BUSHING FROM AN 1 1/4"" X 1 1/4"" LOCK WING VALVE, SAID VALVE BECAME SEPARATED FROM SERVICE RISER AND ESCAPING GAS WAS IGNITED BY UNKNOWN IGNITION SOURCE CAUSING DAMAGE TO (2) SINGLE FAMILY DWELLINGS."
