A 4IN PLASTIC MAINLINE WAS RELOCATED AND THE FUSION MACHINE ON THE FINAL FUSION WELD WAS IN THE PROCESS OF BEING REMOVED WHEN AN UNINTENTIONAL RELEASE OF GAS OCCURRED AND THE GAS IGNITED. THE EXACT REASONS FOR THE FAILURE CANNOT BE DETERMINED WITH CERTAINTY BECAUSE THE EVIDENCE WAS DESTROYED IN THE FIRE.
