Incident Narrative
CONTRACTOR WAS DRILLING POST HOLES FOR THE INSTALLATION OF A FENCE WHEN THEY STRUCK A 2 INCH PE MAIN CREATING A 3 INCH GASH ON THE SIDE OF THE PIPE. THE LINE WAS A ONE WAY FEED DEAD END. THE LINE WAS SQUEEZED AND THE DAMAGE SECTION WAS REPLACED. THE CAUSE FOR THIS INCIDENT WAS THE EXCAVATOR'S FAILURE TO CALL FOR LINE LOCATES BEFORE HE STARTED HIS EXCAVATION.
