"CONTRACTOR WAS OPERATING A TRACK LOADER CLEANING OUT A FARM POND AT 629 HWY D AND HIT THE TOP OF AN 8"" GAS MAIN IN EASEMENT THAT HAD ONLY 4 INCHES OF COVER. THE LINE WAS INSTALLED IN 1963 AND AT THE TIME IT SKIRTED THE OUTER EDGES OF THE POND AND HAD APPROXIMATELY 30 TO 40 INCHES OF COVER. SOMETIME LATER THE POND WAS DUG OUT AND ENLARGED WITHOUT MISSOURI GAS ENERGY'S (PIPELINE OPERATOR) KNOWLEDGE, REDUCING THE COVER OVER THE LINE TO 4 INCHES. MISSOURI ONE CALL WAS NOT NOTIFIED OF THE WORK AND HENCE NO UTILITY LOCATES WERE MADE IN THE AREA."
