**Component or System:** Subaru EyeSight Driver Assist System (specifically the Pre-Collision Throttle Management system). **Incident Description & Warning Messages:** There were no warning lamps, messages, or symptoms of any problem prior to this failure. The incident occurred while I was stopped a
... t a red light at a busy intersection. When the light turned green, I removed my foot from the brake and pressed the gas pedal to proceed, but the vehicle would not accelerate. The system had engaged a false-positive throttle cut. At that exact moment, the car began beeping repeatedly and the EyeSight logo flashed on the dashboard, incorrectly indicating an obstacle or imminent collision, even though the path in front of my vehicle was completely clear. **Safety Risk:** My safety and the safety of others were put at significant risk because the vehicle was unexpectedly immobilized in an active intersection during a green light. This sudden, unwarranted loss of acceleration created a high risk of being rear-ended by the drivers behind me who were expecting the flow of traffic to resume. **Reproduced, Confirmed, or Inspected:** Because this incident just occurred, the problem has not yet been reproduced or confirmed by a dealer or independent service center. Furthermore, the vehicle has not yet been inspected by the manufacturer, police, or insurance representatives.Read more

The thermo control valve (TCV) failed on my Forester (2020 model year). The failure first manifested on 16 June, 2026, with two warning lights: Eyesight Off, and Check Engine. Next day a related failure occurred, described below. Based on my call to dealer, the service department told me the car
... could be driven if the check engine light was steady, not blinking. It was steady. I then attempted to drive 3 miles to a dental appointment, intending to take the Forester to the dealer next. During this trip, automatic front braking spontaneously began. I was able to cross one lane of city traffic and lurch into a parking lot. Tested around the parking lot the next day, the same automatic braking sequence spontaneously engaged. The word “brake” was constantly illuminated on the dashboard. The vehicle could not be driven and was towed to the dealer. Replacement of the TCV led to the disappearance of the warning lights and ended the continuous automatic braking. The critical point is that, had I not had a dental appointment, I would have tried to drive the vehicle to the dealer in heavy freeway traffic when the automatic braking engaged. This would have put me at great risk of being rear-ended, or, if a vehicle stopped behind me, of that person being rear-ended. Subaru never notified me of the pattern of TCV failure, and, instead, internally extended a warranty on the part for affected vehicles for 15 years or 150,000 miles. A recall and replacement is needed to prevent a potentially highly serious accident.Read more

WHAT FAILED: Ford-shipped OTA software (update ID OBCC-AS.AU, "Plug & Charge Improvements," delivered May 10, 2026) generates an admitted false-positive instrument-cluster wrench-light and Ford App service-needed notification. The OTA is installed; vehicle available for inspection on request. FORD'S
... OWN ADMISSION (verbatim, from Ford's published OTA release note): "In some instances after this update, a 'Service vehicle soon' wrench icon may appear in your instrument cluster... You may also receive a Ford App notification indicating service is needed on the vehicle, which you can disregard. Please be assured this is a visual notification only..." SAFETY RISK: The same wrench-light is the documented symptom of NHTSA Safety Recall 25V863 / Ford 25C69 (Park Pawl engagement failure) - active and unrepaired on this VIN per NHTSA's June 2, 2026 refresh (see complaint 11741722). Ford's 25V863 NHTSA filing states a Park Pawl failure displays "A WRENCH LIGHT AND SHIFT SYSTEM FAULT MESSAGE... IN THE INSTRUMENT PANEL CLUSTER" with automatic EPB application and roll-in-park crash risk. Ford has instructed me in writing to disregard the same notification surface that would warn of a genuine 25C69 failure. The two are visually indistinguishable without dealer diagnosis. REPRODUCED/CONFIRMED: Ford itself confirms the defect in its written release note. INSPECTED: No dealer or independent inspection of this OTA-induced false-positive has occurred; Ford preemptively instructed disregard. WARNING LAMPS / FIRST APPEARANCE: The wrench-light icon and Ford App service-needed notification ARE the symptoms. They began with the May 10, 2026 OTA install per Ford's published note. Both are also the warning channels for the unrepaired 25C69 safety condition. I request ODI review of whether OBCC-AS.AU constitutes a defect under 49 U.S.C. 30102(a)(11) when it masks an active federal-recall warning channel on the same VIN.Read more

My 2026 Hyundai Tucson is experiencing a stuck update loop for the 2nd time since I purchased it new from a Hyundai dealer on February 2, 2026. The first stuck update occurred about Feb 18, 2026. A case was opened with Bluelink and it was taken to the deal for diagnosis on March 26, 2026. The update
... loop was cleared by the dealer’s service department in mid April. The second stuck update occurred on or about May 14, 2026, when Bluelink sent out their next update. I am filing this complaint because both times this occurred Verizon’s internet WiFi service was terminated because the provider couldn’t authenticate their service token and that this becomes a safety issue due to the fact that the Automatic Crash Notification and the SOS Emergency Call button are both disabled until the dealer flashes the system software and reestablishes a clean network handshake. The first attempt to correct the issue took about 3 weeks. Currently, I are waiting for an appointment to be made with the deal’s serve department so that they can flash the software again and clear the second stuck update loop. In the meantime, the above mentioned safety features are disabled. I fear that this stuck update issue will be never ending and I will be constantly returning to the deal to have the software flashed and the safety features will be disabled until they do so. All that said, you are aware of the reason why this occurs but continue to allow the manufacturer to flash the software and apply a bandaid. I am referencing NHTSA TSB #25-01-024H (Service Campaign 9C1). I would like to encourage NHTSA to reconsider their position on this ongoing network failure as more than a technical service communication issue but, rather, as an emergency safety recall as many Hyundai customers are experiencing this same stuck software update loop and loss of vital safety features.Read more

While driving, my 2018 Honda Accord suddenly lost power and went into limp mode with no prior overheating or warning signs. The acceleration dropped instantly, and the vehicle nearly caused an accident as it seized up in traffic. Diagnostic scan showed a P0303 cylinder 3 misfire, which is consistent
... with head gasket failure. The issue has been widely reported among other Honda Accord owners with the same engine, and it appears to be a recurring defect. This failure is dangerous because the car can lose power abruptly at highway speeds or during normal driving, putting drivers and passengers at risk of collisions. The vehicle was properly maintained and had no prior indication of internal damage or overheating. The head gasket failure is sudden and mechanical, not caused by user neglect. It needs to be investigated as a manufacturer defect before it leads to serious accidents or injuries.Read more

The car began revving at lights when I was in drive and stopped at red lights. The normal at rest rev is 0.75 however I have seen it as high as 1.5 and had to put the car into park to reset the acceleration. This had been consistently happening over the past month. The first visit to the dealer, I n
... eeded a new battery... that didn't fix the issue. The tech said that after the battery was replaced that the scan showed no error codes. The only codes before the battery replacement were low voltage indicators throughout the car. The next day the same issue happened again. The second scam showed no faults however I asked them to clean the throttle body (which they never suggested) and they did. They said that they recalibrated the new clean throttle body measurement to the computer. For the following 10 days, when I stopped at a light, the tachometer was perfectly stable at 0.75 and there was no "jumping" around like before. Until this day, the acceleration happened at a "stop". However on day 11 after the throttle cleaning, I was SLOWING DOWN to approach a red light and the tachometer revved to 3.0 and I needed to stop immediately and throw it into park. This is so dangerous. I may cause an accident by either lurching forward or needing to hit the breaks to stop the acceleration. There are no error or warning lights that show on the dashboard!!! After the battery was changed, the dealership said they had someone drive it for 10 miles to recreate the issue but found none yet it started up the next day. GROK has been helpful. It suggested first the throttle cleaning which did help, and three other repairs. I'm planning to pay a shop that deals with German cars specifically to scan and evaluate. The car has 43,000 miles on it. This is a testament to German engineering? No longer. I have never seen this in any other of my vehicles. My [XXX] mother in law was told by Subaru White Plains NY her both her front control arms and both front sway bar links had to be replaced due to rust as they were not safe and would not pass the NY vehicle inspection. So I did a Google search and saw 2017 Subaru Foresters had rust issues in this a

Could it have fallen between the cracks, does NHTSA need to have these added as they also obviously have the same issue as they have age to that point of danger. This obviously puts not just 2017 owners at danger, but the public around them. I will try to attach the bills which show what was replaced as unsafe and unable to pass NY vehicle inspection.
