While driving my 2026 Ford Explorer on the highway, the panoramic roof glass made an abnormally loud noise and completely shattered without any impact from an object or other vehicle. No other cars were nearby. The protective shade remained in place and no glass entered the cabin. This appears to be
... a spontaneous failure of the tempered glass. I have already located a nearly identical NHTSA complaint on another 2026 Ford Explorer (Campaign 11734759). The vehicle is currently undrivable in normal conditions and is being repaired through insurance. I am concerned this is a known defect with the panoramic roof glass on 2026 Explorers.Read more

This is for Jeep's brand new Hurricane 4 engine. The buyer has no option to chose the reliable 3.6L 6 cylinder in the 2026 Summit model. Theis 4 cylinders throttle response lag, from a stop or slow speed, is EXTREMELY dangerous! When trying to move into traffic from a street or driveway the Jeep ju
... st rolls slowly as if it was in Neutral; there is no acceleration. I can floor the accelerator pedal & it takes literally a full 2 seconds every time before the engine responds. All the while the traffic you are trying to safely merge into is rapidly approaching. I took my Jeep to the Jeep Service Department & their response was "NO DTCS. COMPARE TO LIKE VEHICLE, AT THIS TIME WORKING TO SPEC . NOTE ENGINE IS TURBO EQUIP AND MAY HAVE A SLIGHT DELAY UNTIL TURBO KICKS IN". No throttle response for 2 seconds does not fall into the category of a "SLIGHT DELAY". When a vehicle is approaching at 45-50+ MPH and you have no throttle response ... it's an accident waiting to happen.Read more

I am reporting what I believe may be a safety defect involving my 2023 Honda Hybrid. I owned the vehicle for less than three years and kept up with all recommended maintenance. During my ownership, I repeatedly experienced sudden tire pressure warnings and rapid drops in tire pressure. Because I was
... concerned, I took my vehicle to a Honda dealership approximately four to five weekends in a row to have the tires inspected. Each time I was told that everything appeared to be normal and no problem could be identified. In March 2026, while driving on the freeway to school, the tire pressure suddenly dropped again. I then heard a very loud popping noise and initially believed that my tire had blown out. Immediately afterward, I lost control of the vehicle and crashed, and my car was declared a total loss. After the crash, I observed that the wheel had turned inward. I was later told by the responding police officers that the wheel had turned inward rather than the tire simply blowing out. I decided to report this because I have learned of other people with the same model who described similar problems involving sudden tire pressure loss or repeated tire warnings despite being told by dealerships that nothing was wrong. While I cannot verify the cause of those incidents, the similarities make me concerned that there could be an underlying safety issue affecting other vehicles. I respectfully request that NHTSA investigate whether there may be a defect involving the tires, tire pressure monitoring system (TPMS), suspension, steering, or another related component. I am willing to provide any additional information requested. Also I don't know if this was the correct channel to go through, I have never reported something like this before. I also send Honda a longer email detailing everything, so I hope they are aware of this issue. Please contact me if you have more questions, I want to help any way I can. The contact owns a 2006 Ford Crown Victoria. The contact stated that while reversing, the vehicle was sluggish. The contact applied more pressure to the accelerator pedal; however, the vehicle unintendedly shifted into drive (D), accelerated, and struck the rear of a parked, unoccupied vehicle while
... blocking oncoming traffic. The contact stated that upon shifting into reverse (R) a second time and depressing the accelerator pedal, the vehicle reversed, but the failure recurred, and the vehicle struck the rear of the unoccupied vehicle, pushing it into another parked and unoccupied vehicle. No warning light was illuminated. No injuries were reported. No Police report was filed. The vehicle was towed to the residence. The vehicle was then driven to AAMCO Transmissions, where the failure was confirmed, but the cause of the failure was not determined upon inspection. The contact was provided an estimate to disassemble the transmission but declined. The vehicle was driven a mile away and towed to the dealer, who informed the contact that the failure was potentially related to the steering column. The vehicle was not diagnosed or repaired, and the contact was referred to AAMCO Transmissions for additional assistance. The manufacturer was not yet contacted. The failure mileage was approximately 108,460.Read more

Without any warning or physical impact, the central infotainment screen completely blanked out and suffered an irreversible hardware failure. This screen acts as the mandatory display terminal for the vehicle's rear-visibility backup camera, as well as the audio generation module for the vehicle's d
... river alerts. Because of this component failure, the vehicle is in direct non-compliance with Federal Motor Vehicle Safety Standards (FMVSS): 1.FMVSS 111 (Rear Visibility): The vehicle completely fails to display a rear-view image when shifted into reverse, creating an immediate blind-spot hazard for pedestrians. 2. FMVSS 101 (Controls and Displays): All critical audible safety alerts, pedestrian alerts, and driver warning chimes have been entirely silenced. The manufacturer (General Motors) and the authorized franchise dealership (F.H. Dailey Chevrolet) have acknowledged the failure and have the part on backorder, but refuse to fully cover the safety restoration under warranty or provide a temporary loaner vehicle. This forces the primary driver, a [XXX] senior citizen, to choose between total isolation or operating a hazardous, federally non-compliant vehicle. This appears to be a systemic manufacturing or design defect inherently tied to the internal HMI module architecture on the late-model Chevrolet Bolt EV platform, rather than individual wear and tear. A formal investigation is requested to mandate a safety recall and enforce full manufacturer accountability. I am filing a complaint against our 2026 Toyota Camry XLE AWD, this complaint probably impacts all new cars. I have no issues with the mechanics or the car, its mileage, etc. However, the so-called safety features, what you call ADAS, are nothing more than a driver distraction. The reason for the c
... omplaint is that there is no means by which these distractions can be permanently disabled. I understand that there was a law suit a number of years ago whose outcome resulted in the distractions being permanently on. However, this is wrong! Adding a disable feature to the system is trivial; and to avoid future lawsuits, only allowing the dealership to activate this feature once the car owner signs a form that prevents them from filing suits is easily accomplished. I urge you to change the rules so these distraction features can be disabled. They are certainly going to cause accidents because the driver pays attention to the car and not that which is happening outside the car.Read more

... A I owned was 2014 had same issue plus drainage hole issues. A panoramic sunroof malfunction can be a safety issue for a few reasons: 1. Emergency situations: In the event of an accident or emergency, it may be necessary to quickly evacuate the vehicle. A stuck panoramic sunroof could prevent passengers from being able to exit through the opening, potentially putting their safety at risk. 2. Ventilation and air circulation: On hot days, being unable to open the sunroof can lead to uncomfortable and potentially dangerous conditions inside the vehicle due to poor ventilation. This can increase the risk of heat exhaustion or dehydration for passengers. 3. Excessive heat build-up: A closed sunroof can also lead to excessive heat build-up inside the vehicle, which can be particularly dangerous for children, elderly individuals, or pets who may be more sensitive to high temperatures. 4. Visibility issues: A sunroof that is stuck closed can also affect visibility for the driver, especially when driving in bright sunlight or in adverse weather conditions such as heavy rain or fog. Overall, a malfunctioning panoramic sunroof can impact the overall safety and comfort of passengers and drivers, highlighting the importance of ensuring that all vehicle components are in proper working condition. Shortly after purchasing this used vehicle from CarMax, the vehicle began experiencing serious mechanical failures involving the engine/emissions system, braking system, and suspension. The vehicle displayed a check engine warning light, and the selling dealer, CarMax Schaumburg, confirmed issues re
... quiring repair, including a faulty oxygen sensor, brake-related repairs, and a suspension link. The vehicle was taken in for service, parts were ordered, and repairs were performed. Despite these repairs, the same issues reoccurred, requiring the vehicle to be returned for additional repair attempts. The problems were therefore reproduced after the initial repair, confirming the existence of unresolved defects. The vehicle was not reliably safe during this period. These failures posed safety risks due to reduced braking reliability, compromised vehicle stability, and engine performance concerns, which could affect drivability and safe operation under normal conditions. The defects were confirmed and serviced by the dealer, as documented through service communications and voicemail messages from CarMax. Warning indicators, including the check engine light, were present prior to repair and prompted service visits. Exact mileage at the time of the incidents is UNKNOWN, as it was not clearly disclosed at purchase. Before the issues were fully and permanently resolved, the vehicle was repossessed, resulting in the loss of the vehicle while unresolved safety and mechanical defects existed. The repossession prevented further inspection or repair by the consumer. The repaired components and service records should be available for inspection through the dealer. No police or insurance inspection occurred in connection with a collision, as the issue involved mechanical and safety defects rather than an accident.Read more

I have a Chevrolet Silverado 1500 with the 6.2L engine. I received the recall notice about the loss of propulsion due to a crankshaft issue. The safety of myself, family, friends and coworkers who ride with me on an everyday basis were put at risk because of the faulty engine. I brought my truck in
... for an oil change in August 2025. I had the Team Chevrolet dealership in Las Vegas Nevada do a telescopic inspection to see if my particular engine had the recall issue. It was discovered the engine did have the recall issue and was kept by the dealership for roughly two and a half months. Chevrolet has confirmed this is a major issue and is affecting roughly 600,000 people. There was no sign of the issue before the telescopic inspection but since the engine has been replaced I have now had three separate issues with misfiring cylinders on the new engine. The plugs and wires have been replaced twice and has not resolved the issue. When the misfires have occurred there were error codes, check engine light notification, and engine sputtering. This again has endangered the safety of myself, family members, friends, and coworkers who ride in my truck on a daily basis. This is now going on four months of issues that keep persisting and I am very concerned that both the manufacturer Chevrolet and the Team Chevrolet dealership are not taking the proper steps in order to solve this very serious issue.Read more

On February 10, 2025 the vehicle, a 2018 GMC Sierra, caught on fire while parked in the driveway of my home. The vehicle had last been driven earlier in the day with a return to home at approximately 12:45pm. At approximately 7pm I heard a loud pop outside of my home and saw orange light coming th
... rough the front windows. Upon going outside, I found the rear of the vehicle engulfed in flames. The fire was coming out around the remains of the license plate, up and around the rear bumper on the left side, and progressing up the tailgate and window hatch on the bed cap. The fire progressed forward over the next few minutes as the fiberglass cap burned. Eventually the fire penetrated the front window of the cap and rear window of the cab and began burning the interior of the cab. About the time the fire department arrived the fuel tank ruptured and began spilling gasoline onto the ground beneath the vehicle. The fuel tank was approximately 7/8 full. The fire department eventually extinguished the fire, but the vehicle was a total loss. My wife and I had been in close vicinity of the vehicle during the afternoon hours and had noticed nothing unusual. There had been no unusual issues with the vehicle during the prior drive, which had been a 5 minute drive to/from a local home supply store. There was collateral damage to the front of my house as a result of the heat from the fire, as well as damage to the driveway. If the vehicle had been parked in the garage it is resonable to assume that the house would have been destroyed.Read more

On [XXX], my Chevrolet Silverado 1500 Custom, Trail Boss 7,500 miles lost immediate power during driving operations. I was accelerating from a stop and my truck immediately lost power and my RPMs rose drastically without any power to my wheels. In an instant, the truck surged in power and continued
... to loose it while in traffic. I had to control the vehicle by slowing down. The check engine light appeared and warning messages to reduce acceleration and drive carefully. All of this occurred violently and without any warning. The truck reduced speed on its own and allowed me to drive 35 mph for two miles into the dealership which happened to be close. There is an extreme risk here for anyone who is driving at interstate speeds and this were to happen. After experiencing this incident, I was told the transmission or the transmission valve body required to be replaced by the dealership. I researched this later and discovered an extensive list of year models with the same problem. I was told services bulletins have been issued on this issue, but this is a serious risk to loss of life. I was told today Chevrolet doesn’t have a solution to the valve body problem and I would be without my vehicle until it is found and repaired. I am requesting a formal investigation. My truck is located at Tom Gibbs Chevrolet, Bunnell , Florida. [XXX] , [XXX] , [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Vehicle has been having more frequent issues starting up. Engine stalls intermittently and hesitates after fueling up. Been getting inaccurate fuel gauge and distance to empty readings lately. Vehicle is available for inspection upon request. My safety was compromised because the car stalled without
... warning on the highway almost causing me to get into an accident, and it also shut down on me after pulling out from the gas station after fueling up. Car also loss power and hesitated on highway when merging, causing traffic to back up. I feel extremely unsafe. Problem was replicated with my mechanic after driving with him from the gas station. The car has always passed inspection from when I bought it and i stay up to date with its maintenance from the dealerships. The Malfunction Indicator Lamp just illuminated a few days ago from 7/18/2024 but have been experiencing this issue slowly for a few months, and has been progressively getting worse until the check engine light came on which is the current situation. Mechanic found multiple codes P1450, P0456 and etc when he connected the computer to my car showing a CPV purge valve and other issues associated.Read more

AUDI 2017 Q7 PREMIUM PLUS: July 25, 2023 - Got a notification "Front right side marker light defective." The car had just passed the 50k manufacturer's warranty. The current mileage at the time was 51,426. Contacted dealership for assistance and they informed there was a recall on this issue but u
... nfortunately my VIN did not qualify for it, also that the vehicle was no longer under the manufacturer's warranty. I then emailed Audi customer support explaining my concern and how I felt this was a safety issue that should be the manufacture's responsibility to repair. I received a call back from them and pretty much what they said was, "we apologize for the inconvenience but unfortunately your VIN does not apply to the recall and the vehicle is no longer under warranty." I got the same response as the dealership. I've taken my car to two different shops and the cost varies from 5k-6k. This is insane for a car that was barely over 50k miles. I've never experienced any other problems with my car in the 4 years I've owned it. This was the very first time. I maintain my car very well and it is always up to date with the maintenance services. This is a safety issue that can affect my safety and others as the purpose of these lights are to inform other drivers about my vehicle when observed from the side. Especially at night. After several attempts with the dealership and no luck I gave up. I found a European Auto Shop to look into this and they are now working with my extended warranty in hopes they can get this fixed for the safety of others and myself. I strongly feel Audi should have made this recall apply to all their 2017 vehicles experiencing this problem. Thank you.Read more
