AIR BAGS:FRONTAL:PASSENGER SIDE:INFLATOR MODULE
Mazda North American Operations (Mazda) is recalling certain 2003-2008 Mazda6, 2004-2005 MPV, 2004 RX-8, and 2006-2007 Mazdaspeed6 vehicles. These vehicles had their passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.
Remedy: Owners are advised not to drive their vehicles until the remedy is performed. Mazda will notify owners, and dealers will replace the front passenger air bag inflator with an alternate one, free of charge. The recall began December 3, 2019. Owners may contact Mazda customer service at 1-800-222-5500, option 4. Mazda's number for this recall is 2618F. Note: This recall supersedes recalls 13V130, 14V362, 14V773 and 15V869.

Mazda North America Operations (Mazda) is recalling certain 2003-2008 Mazda6, 2006-2007 Mazdaspeed6 and 2004 MPV vehicles nationwide. Mazda is also recalling 2005-2006 MPV vehicles in Alabama, Arizona, Arkansas, California, Delaware, the District of Columbia, Florida, Georgia, Hawaii, Illinois, Indiana, Kansas, Kentucky, Louisiana, Maryland, Mississippi, Missouri, Nebraska, Nevada, New Jersey, New Mexico, North Carolina, Ohio, Oklahoma, Pennsylvania, South Carolina, Tennessee, Texas, Virginia, West Virginia, Puerto Rico, American Samoa, Guam, the Northern Mariana Islands (Saipan), and the U.S. Virgin Islands. On July 9, 2019 this recall was expanded to include certain 2004-2011 RX-8 vehicles that were previously recalled under 16V-354 but have not yet been repaired. These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules, and used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the passenger frontal air bag, these passenger air bag inflators may explode due to propellant degradation occurring after long-term exposure to absolute humidity and temperature cycling.
Remedy: Owners are advised not to drive their vehicles until the remedy is performed. Mazda will notify owners, and dealers will replace the front passenger air bag inflator with an alternate inflator, free of charge. The recall began July 6, 2018. Mailing to the affected RX-8 owners began July 6, 2019. Owners may contact Mazda customer service at 1-800-222-5500, option 4. Mazda's number for this recall is 2618F. THE TIRE INFLATION PRESSURE INDICATED ON THE VEHICLE CERTIFICATION LABEL IS NOT CORRECT. THE LABELS SHOW THE MAXIMUM TIRE PRESSURE AS 240 KPA/32 PSI WHEN IT SHOULD BE SHOWN AS 240 KPA/35 PSI. AS A RESULT OF THIS ERROR, THE LOAD RATIOS FOR THE 205/65R15 94H AND THE 215/60R16 95H TIRES EXCEED THE LEVELS PERMITTED BY FMVSS 120.
Remedy: OWNERS WILL BE PROVIDED WITH A CORRECTED LABEL THAT THEY CAN APPLY TO THEIR VEHICLE, OR THEY CAN HAVE A DEALER INSTALL THE LABEL, IF DESIRED. THE RECALL BEGAN ON JULY 16, 2004. OWNERS SHOULD CONTACT MAZDA AT 1-800-222-5500, OPTION #4.

CERTAIN MINIVANS FAIL TO COMPLY WITH THE REQUIREMENTS OF FEDERAL MOTOR VEHICLE SAFETY STANDARD NO. 208, "OCCUPANT CRASH PROTECTION." SOME OF THESE VEHICLES WERE PRODUCED WITH AIR BAG WARNING LABELS INTENDED FOR THE CANADIAN MARKET. THUS, THE LABELS CONTAIN WARNINGS IN BOTH ENGLISH AND FRENCH. AS A RESULT, THE AREA OF THE ENGLISH WARNING IS ONLY 16 SQUARE CENTIMETERS, AS OPPOSED TO THE 30 SQUARE CENTIMETERS REQUIRED BY THE STANDARD.
Remedy: OWNERS WILL BE PROVIDED WITH AIR BAG CAUTION LABELS AND INSTALLATION INSTRUCTIONS WHICH WERE MAILED ON JULY 22, 2004. OWNERS SHOULD CONTACT MAZDA AT 1-800-222-5500.

THERE WERE NO INJURIES AND A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO AN UNKNOWN LOCATION. THE VEHICLE WAS NOT TAKEN TO A DEALER TO DETERMINE THE CAUSE OF THE FAILURE. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND INFORMED THE CONTACT TO SEND DOCUMENTATION FROM THE DEALER. THE FAILURE MILEAGE WAS APPROXIMATELY 166,000.

I WAS ON I-75 WHEN I ACCELERATED HARD WHILE TRYING TO MERGE INTO A GAP IN HEAVY, MERGING TRAFFIC. WHEN I TOOK MY FOOT OFF THE ACCELERATOR MY VAN CONTINUED TO ACCELERATE AS THE ENGINE CONTINUED TO REV AT A HIGH RPM. I STABBED AT THE ACCELERATOR WITH MY FOOT TO SEE IF I COULD UNSTICK IT, BUT IT ONLY CAUSED THE ENGINE TO REV EVEN HIGHER - TO FULL-THROTTLE. I JABBED AT IT MULTIPLE TIMES TO NO AVAIL. I TRIED TO LIFT THE GAS PEDAL WITH MY FOOT BY JAMMING IT UNDERNEATH THE PEDAL AND PULLING IT UP, BUT IT FLOATED FREELY AND CONTINUED TO FALL BACK TO ITS FULL-THROTTLE POSITION. ALTHOUGH I WAS CONFIDENT THAT THE FLOOR MAT WAS NOT THE CAUSE OF THE PROBLEM I PULLED IT AWAY FROM THE PEDALS AND, AS EXPECTED, THERE WAS NO EFFECT. I WAS ABLE TO KEEP CONTROL OF MY VAN BY APPLYING EXTREMELY HEAVY PRESSURE TO THE BRAKE WHILE SHIFTING BETWEEN NEUTRAL AND DRIVE. DURING THAT TIME I WAS STILL JABBING AND LIFTING THE ACCELERATOR WITH MY RIGHT FOOT TRYING TO 'UNSTICK' THE GAS PEDAL. I WAS ENTERING AND CROSSING AND OVERPASS THAT WAS UNDERGOING CONSTRUCTION. CONCRETE BARRIERS LINED BOTH SIDES OF THE HIGHWAY AND BRIDGE WHERE THE EMERGENCY LANES WOULD HAVE OTHERWISE BEEN LOCATED. I HAD TO MAINTAIN SOME SPEED, FOR FEAR OF BEING REAR-ENDED, WHILE SEARCHING FOR THE FIRST OPPORTUNITY TO PULL OVER. MY VEHICLE WAS TOWED TO THE NEAREST MAZDA DEALERSHIP WHERE THE MECHANIC WAS ABLE TO QUICKLY ASCERTAIN THAT THE SCREWING MECHANISM FROM AN UNSECURED AIR HOSE CLAMP HAD SNAGGED THE THROTTLE ARM - THE CLAMP ATTACHES THE HOSE TO THE ENGINE. AS I PUSHED HARDER ON THE GAS PEDAL IT FURTHER EXACERBATED THE PROBLEM BY LOCKING THE ARM IN A MORE WIDE-OPEN THROTTLE POSITION. THE CLAMP HAD BEEN LEFT UNSECURED FROM PREVIOUS WORK DONE AT A MAZDA DEALERSHIP. MY VEHICLE WAS TOWED TO THE NEAREST MAZDA DEALERSHIP WHERE THE MECHANIC WAS ABLE TO QUICKLY ASCERTAIN THAT THE SCREWING MECHANISM FROM AN UNSECURED AIR HOSE CLAMP HAD SNAGGED THE THROTTLE ARM - THE CLAMP ATTACHES THE HOSE TO THE ENGINE. AS I PUSHED HARDER ON THE GAS PEDAL IT FURTHER EXACERBATED THE PROBLEM BY LOCKING THE ARM IN A MORE WIDE-OPEN THROTTLE POSITION. THE CLAMP HAD BEEN LEFT UNSECURED FROM PREVIOUS WORK DONE AT A MAZDA DEALERSHIP. THE ENGINE COILS HAVE FAILED 4 TIMES SINCE WE HAVE OWNED THE VEHICLE. LAST NIGHT WHILE TRAVELING ON THE INTERSTATE IN THE RAIN, KEY WORDS, IN THE RAIN IT STARTED AGAIN. I AM SURE AT LEAST ONE IF NOT TWO COILS WENT THIS TIME. JUST HAD A COIL AND ALL THE BOOTS THAT ARE SUPPOSE TO PROTECT THE COILS AND KEEP THE MOISTURE OUT REPLACED IN FEBRUARY, 2012. AT APPROX. 50,000 MILES MY 2004 MAZDA MPV'S 'CHECK ENGINE LIGHT' WENT ON. WHEN I BROUGHT THE CAR IN FOR SERVICE I WAS TOLD AN IGNITION COIL HAD TO BE REPLACED. THE VEHICLE WAS TAKEN TO THE DEALER WHERE THEY REPLACED THE ELECTRICAL COIL TO THE CYLINDER. THE REPAIRS WERE PERFORMED UNDER FACTORY WARRANTY. THE FAILURE RECURRED WITHIN A YEAR AND THE VEHICLE WAS TAKEN TO AN INDEPENDENT MECHANIC. THE MECHANIC PLACED INSULATING MATERIALS AROUND THE COIL TO PREVENT MOISTURE FROM ENTERING THE COIL. THE MECHANIC ADVISED WHEN MOISTURE WOULD ENTER THE ELECTRICAL COILS; IT WOULD CAUSE A SHORTAGE CONTRIBUTING TO THE FAILURE. IN ADDITION, WHEN THE ENGINE WAS SHUT OFF, THE COOLING FAN WOULD CONTINUE OPERATING. THE FAILURE PERSISTED. THERE WERE NO RECALLS FOR THE FAILURE. THE FAILURE MILEAGE WAS 23,000 AND THE CURRENT MILEAGE WAS 100,000.

WE HAVE 2004 MAZDA MPV WHICH WE BOUGHT NEW. WHILE IT WAS STILL UNDER WARRANTY, WE HAD TO HAVE COILS REPLACED. WE THOUGHT IT WAS ODD THAT WE WOULD BE HAVING TO REPLACE COILS ON SUCH A NEW VEHICLE. AFTER INVESTIGATION, WE FOUND THAT THIS IS A COMMON OCCURRENCE WITH THE YEAR MPV WE HAVE. APPARENTLY THERE IS A DESIGN FLAW WHICH ALLOWS MOISTURE TO GET TO THE COILS, CAUSING THEM TO SHORT OUT. WE MENTIONED THIS TO FITZGERALD MAZDA IN ANNAPOLIS, MD WHERE WE WERE HAVING IT SERVICED. THEY ADMITTED THIS WAS A PROBLEM BUT BASICALLY THOUGHT WE SHOULD BE HAPPY THAT IT WAS STILL UNDER WARRANTY..EVEN THOUGH IT WAS PROBABLY GOING TO BE AN ONGOING PROBLEM LONG AFTER THE WARRANTY RAN OUT. SINCE THE WARRANTY RAN OUT, WE HAVE HAD TO HAVE COILS REPLACED 2 MORE TIMES AT OUR OWN EXPENSE AND ARE WAITING TO HAVE YET ANOTHER COIL REPLACED AS I AM WRITING THIS COMPLAINT. THAT MECHANIC DID EXTENSIVE RESEARCH FOR US AND CAME TO THE SAME CONCLUSION..THIS IS A PROBLEM WITH MOISTURE GETTING TO THE COILS AND WILL HAPPEN AGAIN AND AGAIN THROUGHOUT THE LIFE OF THE VEHICLE. WE ARE NOW DELAYING ANOTHER VACATION..PAYING OUT OF POCKET TO HAVE YET ANOTHER COIL REPLACED. WE WANT MAZDA TO STAND MY THEIR PRODUCT. LIKE MANY OTHER MAZDA MPV OWNERS, I'VE EXPERIENCED PROBLEMS WITH THE VEHICLE'S IGNITION COILS. IN AUGUST 2008, THE VEHICLE WAS RIDING VERY ROUGH AND HESITATING ON THE HIGHWAY. THE DIAGNOSTICS FOR THE CHECK ENGINE LIGHT SHOWED THAT ONE OF THE IGNITION COILS HAD MISFIRED. I HAD THE COIL REPLACED AND, AT THE RECOMMENDATION OF THE DEALERSHIP, REPLACED THE SPARK PLUGS. AT THAT TIME, MY VEHICLE WITH APPROXIMATELY 35,000 MILES ON IT, WAS OUT OF WARRANTY. I PAID $476 FOR THE REPAIR BUT WAS ULTIMATELY REIMBURSED BY MAZDA ON A CUSTOMER GOODWILL BASIS. ONE YEAR LATER, IN AUGUST 2009, THE VEHICLE AGAIN BEGAN RIDING ROUGH AND HESITATING. WHEN THE CHECK ENGINE LIGHT CAME ON, I BROUGHT THE VEHICLE TO THE DEALERSHIP. BASED ON THE EARLIER EXPERIENCE, I SUSPECTED AN IGNITION COIL AS THE CULPRIT. MY SUSPICIONS WERE REALIZED. THE DEALERSHIP FOUND THAT CYLINDER #5 HAD MISFIRED AND THAT SEVERAL OTHER COILS WERE WEAK. THE DEALER INDICATED THAT THE OTHER COILS WERE LIKELY TO GO AND RECOMMENDED THAT ALL OF THEM BE REPLACED. I HAD THE WORK DONE AT A COST OF $960. THE VEHICLE AT THIS POINT HAD ONLY 46,000 MILES ON IT. BOTH REPAIRING DEALERSHIPS MENTIONED THAT THIS IS A COMMON PROBLEM WITH MPVS. FROM MY RESEARCH, I CAN SEE THAT IS A SIGNIFICANT PROBLEM FOR 2002-2006 MPV'S. ENTIRE MESSAGE BOARDS ARE DEDICATED TO THIS MAZDA MPV IGNITION COIL DEFECT. MOST BELIEVE THAT THERE'S A DESIGN FLAW THAT PREVENTS THE COILS FROM DISPERSING MOISTURE PROPERLY, WHICH OVER TIME LEADS THE CYLINDERS TO MISFIRE. IT SEEMS TO ME THAT MAZDA SHOULD HAVE VOLUNTARILY RECALLED THE VEHICLE DUE TO THIS ISSUE. HAVING PREVIOUSLY MADE A CUSTOMER ACCOMMODATION ON THE EARLIER REPAIR, MAZDA HAS REFUSED TO REIMBURSE ME FOR THE COSTS OF THIS LATEST REPAIR. ( 703/527-3000) ADVISED THAT THIS PROBLEM WAS KNOWN TO MAZDA FOR THE PAST FIVE YEARS AND REQUIRED AIR INDUCTION SERVICE EVERY 12,000-15,000 MILES. FRONT WINDSHIELD WIPER ARMS BROKE OFF DURING USE ON HIGHWAY AND HEAVY RAINS. UPON LATER INSPECTION, THE CAST METAL PART OF BOTH ARMS WERE CRACKED NEAR THE HINGE POINT. THE DRIVERS SIDE ARM BROKE OFF AND WAS CAUGHT IN THE PASSENGER SIDE WIPER BLADE FOR A FEW SECONDS CAUSING IT TO SMACK AGAINST THE WINDSHIELD AND CRACKING IT. MAZDA REFUSED TO OFFER ANY ASSISTANCE IN DETERMINING IF THE PARTS WERE DEFECTIVE OR IN OBTAINING REPLACEMENT PARTS. REPLACEMENTS WERE PURCHASED FROM THE ORIGINAL DEALER. *TR

REAR LIFTGATE DOOR GAS STRUT MOUNTING STUD SNAPPED OFF, THERE IS A RECALL IN CANADA FOR THIS ISSUE FOR MODEL YEARS 2000-2004.
