The contact owned a 2023 Lexus RX 350. The contact stated that while the vehicle was stopped at a traffic light, it suddenly accelerated without warning. While driving in a parking lot, the vehicle again surged forward approximately 20 feet. The contact was able to swerve to avoid striking a pedestrian. The vehicle became uncontrollable, and the brakes were inoperable. The vehicle ultimately crashed into a dumpster fence, which fell onto the vehicle. The contact sustained injuries to her mouth, including cuts to the inside and outside of her mouth, and medical attention was provided by a plastic surgeon. The fire department responded to the scene. The air bags did not deploy during the crash. There was no fire, and a police report was not filed. The fire department was on the scene. The vehicle was towed to a local dealer; however, the vehicle was not diagnosed or repaired. The insurance company later deemed the vehicle a total loss. The manufacturer has not been notified. The failure occurred at approximately 18,000 miles.

While driving westbound on Route 70 in Ohio at highway speed, we suddenly heard what sounded like an explosion and immediately felt exposure to outside air within the vehicle. The event was abrupt and very startling while in motion. After safely stopping and inspecting the vehicle, we observed that the driver-side rear door window had shattered. The damage included a pronounced irregular opening located approximately in the middle of the window toward the rear edge, with complete fragmentation of the surrounding tempered glass. We inspected the interior of the vehicle and surrounding area for evidence of a rock, debris, or other projectile but did not locate any object that could have caused the damage. This unexpected window failure created driver distraction, potential risk of injury from glass fragments, and sudden loss of window integrity while driving at highway speed. This report is submitted so that any potential pattern of unexpected automotive glass failure in this vehicle model can be evaluated.

The vehicle was parked on the street next to a residential garage entrance, with no trees or overhead objects nearby. I have photographs documenting the shattered sunroof. No alerts were received on my mobile app at the time. The vehicle remained stationary and was not operated until delivered to the dealership. Around February 2–3, my mobile app indicated vehicle activity, confirming the dealership accessed or connected diagnostic equipment while the vehicle was in their possession. They never disclosed inspection results or technical findings. Dealership and Manufacturer Response February 2, 2026: Vehicle delivered to authorized Lexus dealership; advised to contact manufacturer. February 13, 2026: Lexus North America phone call – warranty/goodwill coverage denied. February 19, 2026: Dealership confirmed repair not covered. Email to service manager requesting inspection findings, diagnostic conclusions, and written denial – no response. February 20, 2026: In-person visit; dealership refused to provide inspection results, stating vehicle was received broken and I could pay for repair or take it unrepaired. Repair Order (R/O) stated: “Probably due to outside influence.” No evidence or technical explanation was provided. Safety Concern The sunroof glass shattered inward, presenting a direct hazard to occupants. The failure occurred within three months of CPO purchase, raising concerns about inspection process and sunroof integrity. I took my vehicle to Lexus of Louisville on 2/2/26 for Recall campaign number: NHTSA 25V744000 (Lexus internal 25LB06. My backup camera does not work at all but I was told my vin is not part of the recall even though my year, make and model is. Lexus charged $250 to diagnose the problem and the repair is $1311 for a new camera. The part is on backorder. I drive on average 25,000 miles per year. Not having a back up camera is a safety risk for myself and everyone around me as I am in and out of a minimum of 12 parking lots per day. My service rep, John said to diagnose the issue they replaced my camera with another one on the lot to confirm that was the issue. I have attached pictures of the problem. I do not believe this is a coincidence and hope Lexus will stand up and recognize the larger issue here.

The 2023 Lexus RX-350 has had complete 12 volt battery failure twice in the last year. The hybrid battery charging procedure for the 12 volt battery is insufficient. Twice I have been stranded. The frequency and duration of the hybrid battery charging is not sufficient. A more powerful 12 volt AGM battery is needed. The dealer says I am not taking sufficiently long trips to charge the battery. Last Saturday I took two 30 - 40 minute trips but the battery had only 11.8 volts requiring a 100% charge. There should be a recall to fix this problem.

He thought this because the trailing arm bolt on the passenger side was very loose and about to fall out. The mechanic replaced the missing bolt and tightened the loose bolt. If we don't drive the car for 4 or 5 days, especially in cold weather, the car will sometimes not start due to a drained 12V battery. We've spoken to Lexus of Rockville, MD and Lexus roadside assistance, who are aware of the issue but say the 12V battery is "healthy." Online research shows this is a widespread known issue with Lexus RX350h and RX500h vehicles from 2023 onward: [XXX] [XXX] . A simple software update to ensure that the high-voltage hybrid battery charges the 12V battery to a greater extent when the vehicle is parked should solve the problem, but Lexus has so far not addressed this problem. On at least 7 occasions the driver was TRAPPED inside the vehicle. The car failed to start and the doors and windows locked. If the driver had not had a cell phone to make an emergency call he would have been locked inside until someone discovered the problem. Had this happened in Phoenix, AZ in the summer, there is a very good chance the driver would have been dead before he was rescued. The last time was December 11, 2025. We replaced the starter battery for 2nd time in 2 years and 40K miles.

When using android auto and I get a phone call the car screen will not return back to it's original screen. If I use apple car play it's just fine and it works like it should and the screen returns to it's original screen. I had the same issue with my 2022 Yukon when I purchased it. General motors later came out with two software updates and after the last update the issue was resolved. I filed a complaint with lexus three different times and they refuse to correct the problem. The safety issue I have is after the phone call I have to keep punching the screen to get it back. I should be watching the road rather than looking and punching the screen. Why should lexus just build their vehicles for apple and not android since 50% of the phones are android. I have a 2024 identical Rx lexus and it has the same problem. General motors seen the software issue and they corrected it but lexus WON'T. Lexus told me originally that it was a android problem and I didn't know any better until my Yukon was updated and fixed. This vehicle was just taken in to lexus and there where no update to fix the problem. Like I said I've been dealing with this on both vehicles since 2023. Like I said I have filed three case complaints and made ten plus phone calls with NO results. The last call was with the corporate office and was told there was no update software update for android phones.

While we were driving, our sunroof just exploded! There was no-one around us. There was a loud bang, glass was flying and the wind was so loud we couldn't hear each other. If my wife would have been driving, we probably would have wrecked because she screamed and through her arms in the air. We exited off the road, inspected our vehicle and took pictures. The only damage we could see was the front section of our sunroof was shattered. We drove to the nearest dealership where they took pictures and ordered a new sunroof. I might add they couldn't guarantee the replacement would be under warranty, which I find shocking. My wife and I are on vacation and we had to leave our car in another state along the way and rent a car.

I do have video proof of the car doing this taken after final attempt for Lexus to review. This video has been sent to Lexus corporate and Lexus Dealer. - Vehicle has been inspected by Lexus dealership as a representative of Lexus corporate. My Dealer has looked at it multiple times and can never duplicate and will not help. I worked with Lexus directly and also were not able to help. I pulled onto a 4 lane divided highway and the car immediately went into limp mode, meaning the car would not go more than 1 MPH even with the gas pedal to the floor. I could NOT accelerate thus my safety was in danger, and I was almost hit by a tractor trailer truck. I pulled over to the side of road as best I could, then shut the engine off and restarted the engine. This apparently re-set some sort of engine control and then the car accelerated normally. The dealer applied an ECU update #24LC03. When I researched this update, it was stated that this was not an actual recall, I wondered how that could that be since I was nearly involved in a fatal accident, and surely there are other similar Lexus owners at the same risk . that I was. The wording of the update described exactly what happened to me. In my opinion this information is too important to shoved under the rug and not made into an actual recall. A life could be saved. When driving, the car engine will randomly accelerate or race. The car does not physically go faster, but the engine revs at a high speed which is very scary. It will continue to accelerate while your foot is off the gas until you either press back down the gas pedal or come to a full stop. The car engine revs extremely loud making the driver feel like the car is going to keep going. I have had the car for 15 months and it continues to get WORSE. I have taken the car 3 times to the Lexus in Freehold NJ when I first bought it to fix this issue. They were able to replicate it in my car but said the tests came back normal and told me this was a "normal reaction" due to the car being a hybrid. The diagnosis was that it was the battery discharging. I wrote to Lexus and the regional service manager, but they have all dismissed me. I filed a complaint with Lexus, but they also dismissed it. I just went on the Lexus Forum site to find many people are now complaining about this issue. What is very interesting is that Lexus has now put out an "Unintended Acceleration (UA) form which asks questions about this issue. LEXUS KNOWS about this major issue but has yet to do anything about it. The other day I approached a few kids playing in the road and started to slow down only to have the car loudly accelerate/rev as I was approaching the children. Very unnerving...something definitely is wrong and needs to be addressed. I was hoping you could help since the manufacturer refused to.

The vehicle hesitates off a full stop such as at a red light. The automatic start stop feature is turned off manually, but car hesitates off the stop creating an unsafe condition if making a left turn where there is no left traffic arrow turn signal and cars are coming in the opposite direction or when entering a moving lane. The hesitation is enough to prevent you from entering or crossing moving traffic unless you wait for all traffic to be gone. This only happens when it is hot outside. When it is cool and air temperature is lower this does not occur and the car operates as normal. Has happened since new and Dealer cannot look into it because there are no failure codes appearing on the car's system.

My RX350 experienced malfunction of the Advanced Driver Assistance Systems (ADAS). Messages regarding the malfunction of the ADAS first appeared around mid-August 2024. Upon inspection during the week of August 27 2024, the Lexus dealer, Akron-Canton of Lexus, indicated that the malfunction was caused by rats chewing on the soy-based wiring. The dealership replaced the wiring at a cost of $1,295.10. This is a problem caused by faulty design and inappropriate parts and design. It is inconceivable that car parts would be made of animal food. This put me and my family at serious risk of accident, as my reliance on warnings was severely compromised. Lexus needs to immediately replace the soy-covered wiring with something that is not edible by rodents or any other wildlife. In the interim, an equitable solution would be to reimburse the actual costs as well as compensate for the risks thrust upon drivers by their reckless use of inappropriate material. It is inconceivable that car parts would be made of animal food. This put me and my family at serious risk of accident, as my reliance on warnings was severely compromised. Lexus needs to immediately replace the soy-covered wiring with something that is not edible by rodents or any other wildlife. In the interim, an equitable solution would be to reimburse the actual costs as well as compensate for the risks thrust upon drivers by their reckless use of inappropriate material. The repair invoice is attached.

My RX350 experienced malfunction of the Advanced Driver Assistance Systems (ADAS). Messages regarding the malfunction of the ADAS first appeared around mid-August 2024. Upon inspection during the week of August 27 2024, the Lexus dealer, Akron-Canton of Lexus, indicated that the malfunction was caused by rats chewing on the soy-based wiring. The sunroof of my 2023 Lexus SUV had a defect and shattered/exploded outwards due to a defect in the tempered glass while driving on the highway. The glass shards fell from the sunroof and cut my face while driving. I had to have a nurse remove the glass shards from my face. Fortunately in this scenario, it did not cause me to lose control of my vehicle on a congested highway. The sunroof defects with Toyota and Lexus vehicles have been documented by several customers online and through several class action lawsuits. The vehicle has been sent to a local Toyota/Lexus dealership for inspection and repair. There were no warnings, messages or other symptoms that the sunroof shattering/exploding could happen.

The contact loaned a 2023 Lexus RX 350. The contact stated that while driving with the cruise control feature set at 65 MPH, the vehicle was rear-ended, hit the median wall, and was then catapulted back into oncoming traffic. The contact stated that the driver's side air bag did not deploy however all the other air bags did deploy as intended. In addition, the contact stated that the front driver-side seat belt failed to secure the driver during the incident. The contact sustained shoulder, hip, leg, and arm injuries and the front passenger sustained left arm injuries. Medical attention was received via the hospital for both occupants. A police report was filed. The vehicle was towed to a tow lot where it was deemed totaled. The manufacturer was not notified of the failure. The failure mileage was approximately 7,000.

The vehicle was taken to a dealer, where the system was reset. The dealer advised the contact that the error message could populate again and that a software fix was being worked on. LANE DEPARTURE: ASSIST Jul 23, 2024
When driving Lexus 2023 RXH with all safety except lane assist features, it suddenly veers to the right under specific conditions below - Have enabled ProActive Driving Assist - The car is travelling on a straight road, and has line markings (yellow left side, white right side) - There is a right turn to the side of the road, and the white shoulder markings taper off and merge to the side of the road (where there is a right turn) The car suddenly veers right and follows the white shoulder marking ignoring the yellow central line and the straight road. Seems very dangerous. Note that Lane Keeping Assistance is disabled (but I checked it since it needs something checked), Adaptive Cruise Contro is not engaged (inner road), but other options are all enabled, including Lexus feature called ProActiveDriving Assist. This is happening especially after the June 2024 firmware upgrade (or after Dealer service)

This 2023 Lexus RX350H and several others vehicles of the same model and year and the 2024 year model have made sporadic roof popping noises that appear to be causes from heat expansion and/or flexion of the vehicle. I have taken this vehicle in a total of seven (7) times to two different Lexus dealerships. The first (4) trips to the dealerships they were unable to duplicate the vehicle, because they didn't keep it long enough. The last three (3) trips to Sewell Lexus in Fort Worth they heard the roof noises and were unable to fix the problem. On the last visit the Lexus Representative supervised the repair and determined that the problem was what the called roofs bows or what appears to be roof supports had come detached from the inside of the roof. The dealership supposedly re-glued the bows/supports and claimed the vehicle was repaired. Three days later the car made the popping noise and has done it often own since then. I have personally driven a similar model loan vehicle from the dealership and it made the popping roof noise also. I have read recent threads this year on the ClubLexus forum site where other Lexus owners of this model vehicle (Both 2023 and 2024) have stated that their vehicle has also made the roof popping noise. I believe this is a Lexus manufacturing defect that they have not been able to fix and have ignored my suggestion that this is a Lexus model defect and also my request for a replacement vehicle, since they haven't been able to fix it. They took the car apart inside to the bare metal the last time they worked on it. This noise has happened when driving, turning corners and when parked and not running. We have encountered 2 instances of Unintended Acceleration. Lexus has inspected the vehicle and states they could not find anything wrong. In both instances the car jumped forward without notice. No warnings were given. The first time it occurred there was over $8,500 worth of damage. No damage the second time which occurred on May 20, 2024.

First Lexus I have ever owned and prob last. I am very disappointed in the paint quality of there cars. I have sustained so many rock chips and scratches from normal driving , no tailgating, or following to closely. Surprised how bad the paint is able to hold. The chips go straight through the paint to the black plastic underneath. My bumper looks like Swiss cheese. I was told by the dealer the design of the car requires it to be PPF. I would have done it if they told me before I drove it 13k.

LEXUS claims the car run 27miles per gallon. But the actual miles I got only 22.1 miles after owning it for a year . THIS IS A BIG LIE. Please take appropriate action!

The rain sensing wiper do not work properly. The wiper swipe once then stop. I have to turn them off and on again to get them to restarted. Also once they do start, they randomly stop for no reason. I have had it to the dealer twice. I was told the dealer contacted Lexus who confirmed this is a problem, with NO fix. The dealer also tested all the new RX 350's models, 2023 and 2024 on their lot, and ALL of them had the same problem. This is a major safety hazard that will cause accidents. It was serviced at Thompson Lexus in Doylestown, PA.

We were driving down the expressway when our panoramic window exploded. There was no one around us, therefore it was not a rock that hit us. Thankfully, we had had the window tinted, therefore the glass didn't shatter around us. We took the car into the service department, and the service man said that it was a rock, but it was not a rock.
