Defect Description

Forest River, Inc. (Forest River) is recalling certain 2017-2018 Rockwood and Flagstaff travel trailers. The slide room arm may contact and damage the passenger side rear tire.

Consequence

The damaged tire may leak air, resulting in a loss of vehicle control and increasing the risk of a crash.

Remedy

Forest River will notify owners, and dealers will install a lift kit to elevate the chassis and replace any tire that has evidence of damage, free of charge. The recall began on November 30, 2017. Owners may contact Forest River customer service at 1-574-642-5164. Forest River's number 10-0588.

Consequence

Forest River, Inc. (Forest River) is recalling certain 2017 Rockwood and Flagstaff recreational trailers. The slide room mounting bracket on these vehicles may contact the driver side front tire, potentially damaging it and causing tire deflation.

Consequence

Tire deflation can increase the risk of a crash.

Remedy

Forest River will notify owners, and dealers will install a rearward offset mounting bracket head to keep the slide room bracket from touching the tire, free of charge. ELECTRICAL SYSTEM:WIRING

Defect Description

Forest River, Inc. (Forest River) is recalling certain model year 2017 Flagstaff and Rockwood trailers equipped with two air conditioners. There is may be excess slack in the wiring harness used in the air conditioner that contains a heating element.

Consequence

If the wires are not tied back and properly secured away from the heating element, the wires may contact the heating element, melt and short, increasing the risk of a fire.

Remedy

Forest River will notify owners, and dealers will install a remedy kit to constrain the wires, free of charge. The recall began on November 1, 2016. Owners may contact Forest River customer service at 1-574-642-5164. Forest River's number for this recall is 10-10062016-0254.

Safety Act Violations

In a Consent Order executed on July 8, 2015, Forest River, Inc. (Forest River) admitted that it violated the Safety Act by: failing to submit accurate early warning reports; not filing certain field communications or reports of Canadian safety campaigns; not providing copies of notices, bulletins, a... nd other communications as required; failing to file certain quarterly recall reports in a timely manner or with complete information; sending notices to dealers without required information; failing to submit communications to manufacturers, dealers, owners and purchasers directly related to a defect or noncompliance; failing to notify vehicle owners and dealers of defects and noncompliances; not timely notifying NHTSA of safety-related defects within five days; failing to conduct timely recalls for Recall Nos. 15V-080 and 15V-156; and failing to respond fully to NHTSA's October 2, 2014 Special Order by the deadline.As part of the Consent Order, NHTSA assessed a total civil penalty of $35 million against Forest River for its violations of the Safety Act. The civil penalty consists of a cash payment and stipulated penalties. Forest River paid the $5 million cash penalty on July 17, 2015. Forest River is required to retain an independent monitor to conduct audits of the company's compliance with the Consent Order and Safety Act. Failure to resolve any issues discovered in those audits will result in portions of the stipulated civil penalties coming due. The Consent Order also contains performance obligations, including retention of an in-house consultant, actions to remedy past noncompliances with the Safety Act, creation of publicly available best practices for Safety Act compliance, employee training, and industry outreach. The population listed in this closing resume is based on Forest River?s untimely recalls and does not include other issues addressed by the Consent Order, which relates to Forest River?s entire business.Based on the Consent Order, Forest River's admissions that it violated the law, and its ongoing performance obligations, this audit query investigation, AQ14-002, is closed. Trailer tire may contact floor

ODI opened RQ14-004 based on 3 Vehicle Owner Questionnaires (VOQs) which reported a condition similar to recall 13V-554.This recall addressed insufficient clearance between the tire and trailer floor.The condition was due to a frame supplier change which resulted in reduced clearance between the top... of the tire and the bottom of the wheel wellOf the three VOQs reported in the opening resume two were not included in recall 13V-554 and one was.Both of the vehicles not included used a different frame than the recalled trailers.Upon thorough examination of the condition experienced by the owners it was found that neither unit exhibited insufficient tire clearance between the bottom of the floor and top of the tire.Forest River's initial Information Request Response (IRR) submission did not adequately address all the questions asked by ODI.Following a meetingwith Forest River personnel on March 20, 2015 an IRR was submitted on May 22, 2015, which satisfied ODI.This submission is included in the public repository.When asked how Forest River identified the recall population they used a production change to determine the start and end dates.On February 3, 2012 Forest River introduced a new frame.The engineering drawings used to launch this new frame failed to identify the frame rail height.Furthermore, Forest River failed to verify that the new frame rail height was the same as the one it replaced.This created a half inch reduction in clearance.This issue was not identified and remedied untilFebruary 4, 2013 which is the end of the recall population. A thorough review of the complaint and warranty informationreceived during this investigation was preformed.After removing claims that could be associated with recall 13V-554 a defect trend was not identified on the remaining population.The only crash which also included an injurycould not be positively associated with the alleged defect.
