EQUIPMENT ADAPTIVE/MOBILITY
MobilityWorks (WMK Inc.) is recalling certain model year 2006-2014 Ford E-150, E-250, and E-350 vans modified by MobilityWorks to be equipped with certain model S2005, S2010, S5005, S5010, S5505, and S5510 wheelchair lifts manufactured by Ricon Corporation. The affected lifts are equipped with folding link arms that may have pivot holes that are too big, allowing the bearings to move out of position.
Remedy: MobilityWorks will notify owners, and Ricon dealers will inspect the link arms for damage or for displaced bearings. The platform will be inspected for cracks. Any damaged parts will be replaced, free of charge. The recall began on December 15, 2015. Owners may contact MobilityWorks customer service at 1-800-769-8267, or Ricon Customer Service at 1-800-322-2884. Note: These vehicles and wheelchair lifts were subject to a similar recall in 2014. All vehicles that received that recall remedy are still affected by this recall as well.

Cummings Mobility Conversion & Supply (Cummings) is recalling certain model year 2006-2014 Ford E-150, E-250, E-350, Chevrolet Express, 2006-2008 Dodge Sprinter, and 2009-2014 Mercedes-Benz Sprinter, and 2013 Ford Transit Connect vehicles manufactured January 1, 2006, to October 8, 2014, equipped with certain model S2005, S2010, S5005, S5010, S5505, and S5510 wheelchair lifts manufactured by Ricon Corporation after January 1, 2006. The platform side plate of the affected wheelchair lifts may crack.
Remedy: Cummings will notify owners to take buses to a Ricon dealer or service center who will install supplemental platform support bumpers. Any platform that has already started cracking will be replaced, free of charge. The recall began January 13, 2015. (Mobility Specialists) is recalling certain model year 2007-2008 Ford E-150 vans modified by Mobility Specialists and equipped with Braun NCL-2, and NVL-2 wheel chair lifts. Over time, and with frequent use, the roll stop latches on the outer barrier of the lifts in these vans may become bent or misaligned.
Remedy: Mobility Specialists will notify owners and will replace the affected parts, free of charge. The recall began in July 2014. Owners may contact Mobility Specialists customer service at 1-877-777-5438.

The vehicles are equipped with certain Ricon S series and K Series wheelchair lifts with an optional armored pendant cable. The armored cable may be positioned such that it does not clear the wheelchair lift.
Remedy: Mobility Works will notify owners and Ricon will provide a supplemental cover kit and instructions, free of charge. This recall campaign is expected to begin July 15, 2013. Owners may contact Mobility Works at 1-877-275-4912 and Ricon at 1-818-267-3000.

FORD IS RECALLING CERTAIN REPLACEMENT FUEL TANKS THAT COULD BE INSTALLED INTO MODEL YEAR 2004 THROUGH 2008 E-150, E-250, AND E-350 VANS AND WAGONS. THE AFFECTED FUEL TANKS HAVE AN EXTRA OBDII SENSOR HOLE ON THE TOP OF THE TANK THAT MAY NOT BE PROPERLY SEALED AND MAY LEAK FUEL.

THE VEHICLE WAS TAKEN TO AN INDEPENDENT MECHANIC, BUT IT COULD NOT BE REPAIRED. THE VEHICLE WAS NOT REPAIRED. THE DEALER AND MANUFACTURER WERE NOT MADE AWARE OF THE FAILURE. THE FAILURE MILEAGE WAS 90,000. THE VIN WAS INVALID.

THE CONTACT COASTED TO THE SIDE OF THE ROAD AND NOTICED BUBBLES ON THE SIDEWALLS AND TREAD SEPARATION ON THE FRONT DRIVER SIDE TIRES. THE VEHICLE WAS TAKEN TO THE DEALER AND ALL FOUR TIRES WERE REPLACED. THE VIN WAS NOT INCLUDED IN NHTSA CAMPAIGN NUMBER: 13T012000 (TIRES). THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE. WHILE APPLYING THE BRAKES TO PARK THE CAR THE WAS A LOUD RIFLE SHOT SOUND, FLASH AND VEHICLE JOLT FROM UNDER THE HOOD. AND THE BREAK WARNING LIGHT CAME ON. DURING REPAIR THE MECHANIC FOUND THAT THE PROBLEM WAS THE WITH THE MASTER CYLINDER CRUSE CONTROL CANCELLING SWITCH. THE SWITCH BLEW APART AND IGNITED THE ATOMIZED BREAK FLUID THAT WAS EXPELLED FROM THE MASTER CYLINDER. FORTUNATELY THE EXPLODING BREAK FLUID SELF EXTINGUISHED AVOIDING AN ENGINE COMPARTMENT FIRE. SINCE THE TRIGGERING OF THIS EVENT WAS THE ACT OF APPLYING THE BRAKES IT WAS ONLY BY CHANCE THAT IT DIDN'T HAPPEN AT HIGHWAY SPEEDS. THE EVENT TOTALLY DISABLED THE FRONT BRAKES AND AT HIGHWAY SPEEDS THE BRAKE FLUID MIST WOULD HAVE LIKELY IGNITED FURTHER BACK IN THE ENGINE COMPARTMENT OR UNDER THE CHASSIS. THERE HAVE BEEN NUMEROUS RECALLS OF FORD PRODUCTS OVER A 20+ YEAR PERIOD FOR THIS SWITCH COMPONENT CAUSING BOTH FIRES AND DEATHS BUT THIS VEHICLE'S S/N WAS NOT ON THE LIST FOR GETTING NOTIFIED. LOOKS LIKE SOMEONE DIDN'T DO THEIR "HOMEWORK" ON THIS ONE, I AM NOT SURE THIS VEHICLE IS SAFE TO DRIVE. I HAVE THE REPAIR BILL FOR THE REPLACEMENT WORK AND THE GENTLEMAN SHOWED ME THE BLOWN OUT SHELL OF THE SWITCH. I PICKED UP ONE OF THE PARTS FROM OUR PARKING SLAB. I HAVE NOTIFIED FORD BY EMAIL.

THE APPROXIMATE FAILURE MILEAGE WAS 53,000.

THE VEHICLE WAS TAKEN TO A PRIVATE MECHANIC WHO FOUND THAT THE CATALYTIC CONVERTER WELDING HAD FAILED AND CAUSED THE PIPES TO FRACTURE. THE VEHICLE WAS NOT REPAIRED. THE APPROXIMATE FAILURE MILEAGE WAS 89,000 AND THE CURRENT MILEAGE WAS 93,000.

TL* THE CONTACT OWNS A 2008 FORD ECONOLINE VAN E150. THE CONTACT STATED THAT WHILE DRIVING APPROXIMATELY 25 MPH, THE VEHICLE STALLED WITHOUT WARNING. THE CONTACT TRIED TO RESTART THE VEHICLE. THE CHECK ENGINE AND ELECTRONIC THROTTLE WARNING LIGHTS ILLUMINATED. THE VEHICLE WAS ABLE TO BE RESTARTED BUT WHEN THE FAILURE WOULD RECUR WHEN THE ENGINE RPMS INCREASED OVER 2,000. THE VEHICLE WAS NOT TAKEN TO A DEALER. THE MANUFACTURER WAS NOTIFIED OF THE PROBLEM. THE APPROXIMATE FAILURE MILEAGE WAS 66,325.

I HAD TO REPLACE ALL 5 TIRES WITHIN 2 YEARS OF MY VEHICLE PURCHASE. THE NEW TIRES ARE NOW JUST OVER 2 1/2 YEARS OLD. ONE BLEW OUT ON A RESIDENTIAL STREET WHILE GOING 35 MILES/HOUR. THE 2ND TIRE REPLACED WAS ALSO SEPARATING AND CAUSING THE VAN TO VIBRATE JUST AS THE FIRST ONE SO I TOOK IT IN 6 WEEKS AFTER THE FIRST BLOWOUT. LEAVING THE TIRE STORE MY VAN IS STILL VIBRATING AND NOW I NOTICE THE 3RD TIRE IS BUCKLING ON THE OUTSIDE WALL. THE 4TH TIRE DOES NOT APPEAR TO HAVE ANY MAJOR ISSUE YET. THE 5TH IS MY SPARE-NEVER USED. DISCOUNT SAID THEY WOULDN'T REPLACE THE 2 FRONT TIRES UNTIL THEY HAD THE SAME ISSUE AS THE REAR TIRES, WITHOUT COST TO ME. THESE TIRES ARE FROM THE SAME LOT #S. MY AGENCY OPERATES 7D VEHICLES THAT TRANSPORT YOUNG CHILDREN. THE SEATS IN WHICH WE MUST PLACE REAR-FACING INFANT CAR SEATS DO NOT ALLOW US TO PROPERLY SECURE THE CAR SEAT. THERE IS ONE TETHER INSTALLED ON THE BACK OF THE SEAT AND 2 INSTALLED WHERE THE CUSHIONS MEET. THE STRAP THAT IS SUPPOSED TO CLIP TO THE TETHER ON THE BACK OF THE SEAT IS TOO SHORT FOR REAR FACING CAR SEATS. THE CAR SEAT CAN BE SECURED USING THE SIDE TETHERS, BUT THE INABILITY TO SECURE TO THE REAR TETHER RESULTS IN THE CAR SEAT MOVING FORWARD & BACKWARD. IF THERE WERE A FRONT-ON COLLISION, THE INFANT'S HEAD WOULD THRUST FORWARD. I AM NOT SURE IF THIS IS A PROBLEM WITH THE VEHICLE DESIGN OR THE CAR SEAT.
