Per my phone call today(2026/05/13)to [XXX] @ [XXX] (PhoneReference #XXX): I have written an explanation which is too lengthy to fit herein. I shall email it to [XXX] AFTER filing this form and receiving a Written Form Reference #. I shall supply the Written Form Reference # in the email. Summary of 5 above specific requests: 1. Car did not accelerate, engine semi-stalled (although
... did not stop running completely). Could be engine, transmission, electrical, cruise-control, other, or some combination of two or more of the foregoing. Dealer may still have the car, or it may have been sold/disposed-of to a party unknown to me. 2. Fortunately, since I did NOT test pulling out into traffic, and waited till highway was mostly clear prior to testing acceleration, no one was at risk. However, the problem is that there ARE instances where risk of serious-injury/death is real. 3. No. Salesperson later reported that Dealer felt that there was no problem. Consequently, I believe the Dealer was unable to reproduce the problem. 4. Not to my knowledge. 5. No. P.S. The below form asks for speed, but does not accept various speeds, which were 0, about 20mph, about 30mph. Purchased my 2011 Subaru Outback 3.6R Limited from the dealership, new. Since 2013, the Subaru dealer has discovered multiple oil leaks, replaced the head gaskets 3 times, thermostat 4 times, the alternator 3 times, timing belt 4 times, radiator 3 times, starter once, and my HVAC system twice...all due to my engine seizing multiple times while driving on freeways. No warning lights, just the engin
... e seizing suddenly. This caused a few minor bumper accidents on the freeways because of these incidents. Nothing was reported because no one was injured. Last year the engine was replaced with a used/refurbished one by the dealership. In less than 11 months after this engine was replaced, the dealership states I'm having multiple oil leaks again, most originating from the cam carrier. Now, my engine is struggling on slight grades. The newly installed used engine only came with a six month warranty and now Subaru states I need another engine replacement. Money is tight, so for now I'm keeping a pan under my car to catch the oil and I will keep topping off the oil every week.Read more

Driving downhill at 70mph on highway. crossed from #2 lane to #1 lane. I did not hit any potholes, I believe I just drove over the "bot dots". This started the "death wobble", the uncontrolled oscillations in the front suspension/tires. I had to take my foot off the accelerator and let the vehicle come down to about 35mph before the violent shaking stopped. I could reproduce this on my own by hitt

While driving at highway speed in 6th gear, the vehicle unexpectedly downshifted into what appeared to be 1st gear without warning. Immediately afterward, the vehicle falsely indicated that it was in Reverse while still moving forward, and the backup camera activated during travel. The sudden downshift caused abrupt deceleration and created a significant safety concern due to the risk of loss of v
... ehicle control and the possibility of being rear-ended by surrounding traffic. The issue has occurred multiple times and temporarily reset after shutting the vehicle off and restarting it. During the most recent incident, the vehicle also experienced a delayed restart. The vehicle was inspected by a Ford dealership, which retrieved diagnostic trouble codes P0706, P0707, and P1702 related to the transmission range sensor system. The dealership diagnosed failure of the molded leadframe/internal harness in the transmission main control, causing transmission range sensor faults and erratic shifting behavior. The dealership advised that the repair would cost approximately $4,000 and stated that the repair is not currently covered under the active Ford transmission recall. Prior to the failure, symptoms included intermittent erratic shifting behavior and false gear indication. No collision occurred, but the vehicle behavior created an unsafe driving condition at highway speeds. The failed component should be available for inspection through the servicing Ford dealership.Read more

On May 13, 2026 at approximately 2:50 PM, I experienced an unexpected activation of the EyeSight pre-collision braking system in my 2026 Subaru Crosstrek while pulling into the Chick-fil-A entrance area in [XXX] . The vehicle suddenly applied hard braking even though there was no imminent collision in front of me. The sudden braking created a hazardous situation and nearly caused me to swerve tow

At the time of the incident, there were no warning lights or prior indications of a malfunction. The vehicle currently has approximately 900 miles on it. I have since contacted the Subaru dealership and scheduled a warranty inspection of the EyeSight/pre-collision braking system for further evaluation. I am writing to dispute the denial of my paint defect warranty claim formally and to demand that Hyundai Motor America reconsider its decision. My 2018 Hyundai Ioniq (Silver, VIN: [XXX] ) is exhibiting significant paint bubbling — the identical defect that Hyundai has already acknowledged and accepted responsibility for in white-painted vehicles across multiple model years. Timeline of Events
... • I first contacted Hyundai Customer Care by phone, and was directed to visit an authorized dealership. • On May 8, 2025, I visited Tustin Hyundai, where Dealer Coordinator Alex Cervantes documented the paint bubbling by photographing the affected areas and submitted the claim to Hyundai Motor America on my behalf. Dealer Reference #: XXX. • On May 11, 2025, Alex called and mentioned that Hyundai has denied my claim. Basis of My Complaint Hyundai has publicly acknowledged that paint bubbling and delamination affecting certain 2017–2018 model-year vehicles constitute a manufacturing defect, not owner negligence or environmental damage. In response, Hyundai issued Warranty Extension TXXW and subsequently the broader Z05 extension (10 years/unlimited miles), covering affected vehicles with white paint. The paint defect on my silver 2018 Ioniq is identical in nature and presentation to the defect that Hyundai has already accepted responsibility for. The bubbling is occurring on body panels — not due to collision damage, improper maintenance, or environmental factors — and is consistent with the delamination pattern described in Hyundai's own Technical Service Bulletins. Denying my claim solely based on paint color, while acknowledging the same underlying manufacturing defect in other colors, is inconsistent and Hyundai not accepting responsibilty. Check engine light came on and I noticed that the cooling fan would continue running for several minutes even after turning vehicle off. Had a mechanic run the diagnostics and the code came back as a thermostat/cooling issue. Although when I called to make an appointment and told him the issue and that it was a 2021 Traverse, he knew exactly what the issue was. He said that is a very common issue
... in all 2021 Chevy/GM engines. Online research provided a couple special coverages for repairs on 7 different GM vehicles from that same time period. Of course, the Traverse is one that is not included. Seems fairly obvious this was a GM engine issue from all vehicles in the 2021-2023 timeframe. Not sure how they can only pick out 7 models, but I am hoping an update can be made to add the Traverse and any other GM models that have that same 2021 engine.Read more

Kline Volvo of Maplewood, MN confirmed the vibration and performed the TJ-prescribed repairs: tire balancing, tire replacement, and driver's airbag module replacement containing an oscillation damper. Volvo's own TJ notes this repair only "lessens" the vibration. The vibration improved but in my assessment has not been fully resolved. A second Volvo dealer's service manager independently acknowledged the issue and consulted Volvo's regional service representative, who confirmed no further fix is currently available beyond the TJ repair. I have escalated to Volvo executive leadership in writing and am awaiting response. The vehicle is available for inspection.Read more

I took it in and they determined it was the clockwork-steering column control mod. I have already gone ahead and paid for the parts which are back ordered and waiting on repairs. When this incident occurred I did not see or notice any prior warnings. Once I received the information of the issue I did research and found that trucks of the same year And model have been been recalled for this issue, however I did not see mine listed when I ran the vin. My truck has had 6 other safety recalls and I believe this is another recall that should be addressed.Read more

Vehicle has an open safety recall involving a condition that may increase the risk of a crash. During a visit to an authorized dealership for information regarding the recall, dealership personnel advised against completing the recall procedure. I was informed that due to the age of the vehicle and components, performing the recall could potentially damage the headlight seals and that I could be f
... inancially responsible for repairs if damage occurred during the recall process. This raised concerns because the recall relates to a safety issue, yet I was discouraged from having the repair performed unless I accepted potential liability for damage resulting from the recall work itself. As a result, the safety recall remains unresolved. No warning lamps or messages were present prior to this interaction. The issue described here concerns the handling of the recall and the guidance provided by the dealership regarding completion of a safety recall repair.Read more

Purchased my 2014 Jetta TDI SportWagen DSG used with 60k miles in 2017 from a Subaru dealership. It was purchased at auction by the dealership as reported at auction known to be an unmodified dieselgate vehicle. The dealership performed the modifications to bring it into emissions compliance and sold it to me. I noticed an occasional "boing" sound from the back and my wife wouldn't drive it, sh
... e said it was too twitchy on the highway. I just thought that was part of the Sportwagen experience. In 2026 I took it to my local garage for a [XXX] state inspection. Mechanic showed me that the front right coil spring was broken as part of the strut, and both rear coil springs were also snapped. The rear spring pieces "screwed" up into themselves and it was actually difficult to see the snapped section. I had been driving for a year on a single front left spring, the only remaining good spring. Total cost of repairs was $4500 in parts and labor (which was actually quite reasonable for the work performed). From [XXX] I purchased VW OEM "Towing" springs for the rear and new front struts and new rear shocks, plus all linkages, mounts and all bolts plus a $150 alignment. Regarding the engine, the glow plugs were replaced in 2025 and the dissimilar metals had "galled" the threads and the head had to be chased with a reamer in order to install new glow plugs to the correct torque. The faulty glow plugs were determined to be VW OEM parts from the factory. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Component/System Failure: Total failure of the Autopilot ECU (DAS Computer) and the Front-End Module (FEM) wiring harness. Incident Description: My 2021 Tesla Model 3 (37,000 miles) experienced a catastrophic and "silent" failure of the entire vehicle vision suite and all active safety systems. A Tesla Service technician (Warrington, PA) inspected the vehicle and confirmed the root cause was a fa
... ctory-defective (crimped) internal coaxial cable in the wiring harness. This manufacturing defect caused an electrical short that destroyed the primary safety-critical computer. Safety Risk: This failure resulted in a direct violation of FMVSS 111 (Rear Visibility), as the backup camera was rendered completely inoperable. Furthermore, all collision-avoidance features—including Automatic Emergency Braking (AEB), Forward Collision Warning, and Blind Spot Monitoring—were disabled. Critical Safety Concern (Silent Failure): The vehicle provided zero driver-facing alerts, dashboard warnings, or lamps to indicate that these safety systems were offline. I unknowingly operated a vehicle that was non-compliant with federal safety standards and lacked all primary crash-avoidance technology. The failure codes (APP_w045 and VCFRONT_a158) were only discoverable by manually entering the vehicle's "Service Mode." Manufacturer Status: Tesla Service has confirmed the hardware failure and the wiring defect but is currently refusing to cover the $2,800 repair cost, despite the failure involving systems currently under federal recall (SB-26-00-016). I am reporting this because a factory wiring defect causing a total, silent failure of federally mandaRead more
