I am reporting a safety-related concern involving the right front axle on my 2024 Honda Prologue. When making right-hand turns at low speeds, the vehicle produces a loud clicking noise coming from the right front axle/CV joint area. The noise is repeatable and occurs consistently during turning maneuvers. I reported this issue to my authorized Honda dealership. On 05/21/2026, I dropped the vehic

I am reporting a serious safety defect in my brand-new 2025 Toyota Land Cruiser (Hybrid J250 series). Vehicle Information: VIN: [XXX] Mileage at time of failure: approximately 4,000 miles Date of purchase/delivery: [XXX] Purchased new from: Koons Toyota Tysons Corner, VA Problem Description: On [XXX], while driving, a suspension component detached/fell off, causing loss of control and a cr
... ash. This was a complete mechanical failure of the suspension system with no prior warning or impact from another vehicle. The failure occurred despite the vehicle having extremely low mileage and despite me previously reporting suspension issues to the dealer. Prior to the crash, I took the vehicle to the dealer on April 17 , 2026 for suspension problems. The dealer inspected it and explicitly stated that the vehicle was safe to drive. Shortly afterward, the suspension failed catastrophically, resulting in the crash. This indicates a manufacturing, design, or assembly defect that poses an unreasonable risk of crash, injury, or death. No modifications, abuse, or lack of maintenance contributed to this failure. This is a new 2025 model with no prior owner history. I believe this is a safety defect that may affect other vehicles, especially given the extremely low mileage at failure. I request that NHTSA investigate this as a potential safety defect, consider opening a defect investigation, and issue a recall if warranted. I attached letters i sent to dealer service advisor and service manager and i haven't gotten respond at all. I mistyped date for service and service was done on March 17th, 2026. I am willing to provide additional evidence, including the failed component for inspection if needed. Please contact me at [XXX] or [XXX] for any questions or to arrange inspection. The Lexus TX 500h is equipped with a 6-speed sequential-shift automatic transmission, which is paired with the Lexus DIRECT4 all-wheel drive system. This setup utilizes a 2.4-liter turbocharged hybrid powertrain to deliver 366 horsepower and 406 lb-ft of torque. During many occasions, the transmission on the 2026 Lexus TX500h would shudder during acceleration or deacceleration. I noticed it hap
... pening around 25-40mph mostly and it has happened more than once. I have demonstrated the issue to the dealer on Wed, May 20th around 10:15am Pacific but they didn't know how to address the issue. I did a ride along with the service tech from Lexus of Pleasanton and it happened again during acceleration. The dealer noted that there were no error code when they scanned the system and no observed leaks. I have a video clip but was unable to upload it. The issue with transmission shift thump when this issue happen feels like the transmission locked up. It would shake the vehicle and lose power while accelerating before it starts to gain power back in 1-2 seconds. This causes a safety issue since that pause in acceleration thus this can be dangerous while merging onto a freeway or areas with lots of traffic. I noticed the same issue on some occasions, it would shake during deceleration as well. This issue is intermittent. Maybe Lexus/Toyota can address this with the transmission shifting update. IMO, It's a safety issue that needs to be looked and fixed by Lexus/Toyota. Dealer asked me to come back only. They reset my information screen to address a sliding driving issue where when the power is turned off on the vehicle the seats will go back and immediately go forward.Read more

Vehicle: 2019 Hyundai Santa Fe, VIN [XXX] , exterior Quartz White Pearl. Component: Exterior factory paint (paint/trim). Complaint: This vehicle exhibits a systemic, progressive factory paint defect — excessive peeling and bubbling of the white finish that spreads from one body panel to the next. This is a known defect across this vehicle generation; Hyundai acknowledged it by issuing Warranty
... Extension Z05 (TSB 25-BD-004H), which extends paint coverage to 10 years/unlimited miles for affected white-paint vehicles including the 2019 Santa Fe. Hyundai repaired the hood under this warranty in 2025, during which the vehicle was out of service for 77 consecutive days. Since that repair, the defect has migrated to the adjacent fender and has also appeared on a door, demonstrating that it is spreading across the vehicle. Hyundai has offered to repaint only the single currently-visible panel rather than addressing the systemic defect. I am filing this complaint to document the defect and its progression, and to support the public record regarding factory paint failure on these vehicles. No crash, injury, or fire has resulted. While driving the vehicle it lifts from the back and lowers in the front and the car while in drive malfunctions leaning to the right applying pressure on the front tire and cause the tread on the tire to come off which was stated as a defect with no way to fix or resolve at this time, I was told by the dealership that the tires are not under warranty and my question was if the car is malfunctioni
... ng causing tread wear only on the front passenger tire wouldn’t that be covered by the dealer to replace the tire. I was then told that they are having the same problem with the new hummers and that if the tread was worn in the first 12,000 miles of Owership that it was due to lifting the vehicle above the default setting and hard driving that caused damage to the tires but they stated that it had damaged both front tires in the hummer, so they said the Escalade had the same problem but here’s the issue we don’t have a button to raise the vehicle up and this was caused by the computer sensor lowering the car while driving which I have no control over when it lifts or lowers and it’s only damaging the right front tire which proves that it’s a defect and GM said they were not responsible for the tires and would not escalate this situation so that my car could be addressed because of this issue and the text at the dealership gave me a copy of the document stating about the hummer issue and agreed with me that my car has a defect but could do nothing without gms ok so now I’m left to drive a car that leans on its own and may blow out my passenger tire at any time and that is beyond my control.Read more

My 2022 Chrysler Pacifica Hybrid has developed a dangerous rear brake system malfunction. During a dealer inspection on 4/27/26, the certified technician confirmed that the rear brake pads are only contacting approximately 50% of the rotor surface, leaving the remaining half completely unused and heavily corroded. This is not normal corrosion or wear. It is a mechanical failure in which the pads a
... re not engaging the rotor correctly. The technician also stated that the pads were sticking in the caliper slides, which prevented full engagement. This is a functional defect in the brake hardware, not environmental corrosion. Corrosion does not selectively prevent half of a brake pad from engaging the rotor unless the caliper mechanism is not operating properly. This defect results in: Reduced rear braking force Increased stopping distance Excessive load on the front brakes Potential loss of stability during emergency braking Possible degradation of the parking brake The vehicle was serviced on 9/23/25 (tire rotation + NYS inspection) and 12/10/24 (four new tires + NYS inspection). No rear brake issues were identified at either visit. If this were normal corrosion, it would have been visible during those inspections. The sudden onset of severe partial engagement by 4/27/26 indicates a latent mechanical defect, not gradual wear. Chrysler/Stellantis has refused to classify this as a defect and instead labels it “corrosion,” despite the technician’s findings of pad binding in the slides and improper brake engagement. The manufacturer’s internal system blocks any warranty or engineering review because it incorrectly categorizes the failure as routine maintenance. This is a premature and unsafe brake system failure at only 33,000 miles. The braking system is not operating as designed, and the defect could lead to reduced braking performance or loss of control. I am requesting NHTSA’s review of this issue as a potential safety defect affecting the rear brake systemRead more

Additional issues were communicated through technician video and text, including a potential oil leak and possible fluid mixing, but none were documented or diagnosed in the service record. A dealership manager later stated that the vehicle was “not safe to drive,” but no written explanation or safety guidance was provided, and the vehicle was released without clarification. No diagnostic records, technician notes, or findings have been provided by either the dealership or GM despite escalation. The consumer has worked through appropriate dealership and manufacturer channels for approximately one month without resolution. The consumer is now being directed to return the vehicle for additional diagnostics at the consumer’s expense without defined scope or explanation. The vehicle remains with an unresolved condition and an open safety recall, without clear guidance regarding safe operation. This lack of clear, documented information is particularly concerning for senior-aged owners who rely on accurate guidance to make safe decisions.Read more
