Incident Narrative
A temporary employee entered a work area to inspect bolts for a piping system when they fell into a 3-by-4-foot hole. They fell more than 25 feet, striking pipes before landing on concrete. The employee was hospitalized with left hand and knee fractures, spinal and neck fractures of the C2, C6, C7, T2, T3, and T11 vertebrae, and a laceration to the skull that extended to the right eye. Fall protection was in not place at the time.
