Incident Narrative

An employee was resuming re-roofing operations and walking on the lower roof. His anchor plate detached from the roof causing him to lose balance and fall approximately 10 feet to the surface below. The employee suffered fractures to their tibia, calcaneus, fibula, and L1 vertebra as well as renal insufficiency.
